摘要

本文探討照護一位因乙狀結腸癌轉移至膀胱手術患者的照護經驗。照護期間自2012年11月29日至12月27日，筆者運用Gordon十一項健康功能型態作為評估工具，經由觀察、會談及身體評估等技巧，收集個案之相關資料，整理分析後確認個案有三項護理健康問題：急性疼痛/與手術後傷口有關、焦慮/與對疾病預後及治療的不確定感有關、自我照顧能力缺失/與導管留置、手術傷口有關等。在照護過程中，個案因癌症及後續相關的治療對其造成生理及心理各方面的衝擊，筆者護理過程中，扮演傾聽、支持及諮詢者角色與個案建立信任的護病關係，提供完整的資訊，使其對疾病有正確的認知，協助個案面對疾病及支持系統的建立，讓個案能有正向的調適降低不確定感，使其情緒得以抒發，減緩個案疼痛不適，並且指導個案及家屬面對疾病自我照護之相關知識，增進自我照顧能力及適應能力進而提升生活品質。

This article discussed a nursing experience in caring for a patient underwent surgery of sigmoid cancer with bladder metastasis. The nursing period was from November 29th to December 27th, 2012. As assessed by Gordon's 11 functional patterns, the data was collected by observation, interview and physical evaluation. He had three health problems: Anxiety related to the uncertainty about healing and treatment, Acute pain caused by the wound after surgery, and Self-care deficit. The patient suffered severe both physiological and psychological impact. The author established a trust nursepatient relationship by listening, supporting and consulting to help the patient face up to the disease as well as build a support system. The patient could have a positive adjustment to reduce uncertainty and gain knowledge to enhance self-care ability and adaptability and to improve the quality of life.
