本文是探討一位23歲年輕女性因卵巢癌復發合併肝轉移,需再次手術及化學治療之護理經驗。過程中病人對於疾病再次復發感到焦慮、無助,深怕治療沒有成效、造成家人負擔,進而引發筆者的動機。護理期間為2018年3月13日至2018年4月14日,共33天,藉由傾聽、訪談、觀察、會談、身體評估及醫療團隊討論等方式收集資料,以Gordon十一項功能性健康型態進行整體性護理評估,發現病人主要的健康問題有化學治療後潛在危險性感染、急性疼痛、疾病焦慮。病人對於治療結束後半年就疾病復發,顯得焦慮不安,表情凝重,很少主動說話,多數時間閉眼休息、不願與他人對答,護理過程中,筆者以同理心及真誠的護理態度,與病人建立信任的護病關係,提供完整的資訊,促使病人能了解手術的過程,引導表達對於手術及後續化學治療的焦慮情形,並藉由主動關懷、傾聽、陪伴,使病人能在護理人員及家屬的支持陪伴下,順利的完成後續治療。期望此照護經驗分享,可提供臨床護理人員照護類似病人之參考。
The article described the experience in caring for a 23-year-old young woman suffering from recurrent ovarian cancer with liver metastasis. During this period, the patient felt anxious, helpless and worried that she might result in her family's burden if her treatment may fail. The author collected data by the means of listening, observation, conversation, physical assessment and medical care team discussion from March 13 to April 14, 2018. By adopting Gordon 11 functional health assessment tool, the author has identified the patient's main problems, including risk for infection after chemotherapy, acute pain and the feeling of anxiety in illness. During the nursing care process, the author used the concept of empathy and sincerity to build the relationship with the patient and provided information regarding the surgery and chemotherapy. Finally, the patient has completed the treatment successfully after receiving the support from nursing staff and her family. Through sharing nursing care experiences, the author hoped to provide this information as a reference of the nursing process in caring for the patient with recurrent ovarian cancer.
