提升手術室器械清洗流程完整率
Enhance of the Completion Rate of Equipment Cleaning Process in the Operating Room
摘要
確保器械清洗完整是落實感染管制、提升手術安全重要方法之一，本專案旨在提升手術室器械清洗流程完整率。調查後發現，手術室器械清洗流程完整率僅56.0%，人員滿意度僅6.7分，認知得分僅53.2分。原因為手術間隔時間短、拆卸清洗耗時、氣槍太遠、泡酵素耗時、忙碌、器械清洗刷難用等。經修正器械清洗作業規範、簡易清洗流程口訣、更改氣槍位置、增購合適器械清洗刷、新增器械清洗專用三水槽、增設器械清洗人力、持續監測及在職教育、納入新進人員學習項目等措施後，手術室器械清洗流程完整率提升為100%，人員滿意度提升為9.2分，認知得分提升為95.6分。故整合器械清洗流程及設備，確實可提升手術室品質和滿意度，且作為類似單位參考。

Thorough cleaning of surgical equipment is one of the important measures to implement infection control and improve surgical safety. The aim of this proposal was to increase the completion rate of equipment cleaning process. The initial survey revealed that the completion rate of equipment cleaning process was 56.0 percent; the scale of staff satisfaction was 6.7; and the staff awareness score was 53.2 points. The reasons for the low completion rate included short turnaround time between surgical cases, complexity to disassemble device, long distance to reach air jet cleaning machines, long time of enzymatic soak, being busy with other duties, and awkward instrument cleaning brushes. A series of improvement measures were adopted: revision of equipment cleaning protocol, simplification of cleaning mnemonics, relocation of air jet cleaning machines, purchasing appropriate instrument cleaning brushes, adding cleaning modules with triple sink configuration, hiring more cleaning staff, continuous monitoring and on-the-job training, and incorporation of cleaning practice in the orientation program. Following these measures, the completion rate of equipment cleaning process has been raised to 100%; the scale of staff satisfaction has increased to 9.2; and the staff awareness score has increased to 95.6 points. In summary, the better integration of equipment cleaning process and facility improves quality and efficiency of operating room instrument availability and enhances staff satisfaction. The proposal may be used for the reference of other units with similar needs.
