一位早產兒母親併發腦動脈瘤破裂在加護病房早期復健之護理經驗

Nursing Care of a Mother who has a Premature Infant and Suffered from Rupture of Cerebral Aneurysm Requiring Early Rehabilitation in Intensive Care Unit

摘要


本文探討一位非預期緊急娩下早產兒之母親,產後第二天因突發腦動脈瘤破裂,緊急頭部手術後入加護病房,意識清醒後面臨身體活動功能障礙,而自認無法做好母親之角色。筆者於2015年3月5日至3月13日加護病房護理期間,經由觀察、會談、查閱病歷等方式收集資料,應用羅氏適應模式理論為結構,確立個案護理問題有:身體活動功能障礙、呼吸清除功能失效及親職功能障礙。在護理過程中,早期介入呼吸訓練及心理支持,提供整體性、個別性需求之照護使呼吸器成功脫離;運用團隊合作共同擬定復健計劃,協助恢復肢體最大功能;提供早產兒訊息以協助正向親職關係建立,在後續出院追蹤個案能提升自我照顧能力及發揮母職的角色,期許此照護經驗能提供護理同仁在未來照護此類病人之參考並能提升照護能力。

This article is to report the nursing care of a mother who labored a premature infant and on the second day after delivery, she suffered from sudden onset of cerebral aneurysm rupture and went through cerebral operation prior to admitted to intensive care unit. After her consciousness has recovered, her activities of daily living needed to be assisted as the result of impaired physical mobility. Therefore, she presumed she was not able toper form the role of motherhood. The periods of nursing care was from March 3 to March 13, 2015 in an intensive care unit. Data were collected by observation, interviews and chart review. Based on the Roy's adaptation model, the health problems identified were: impaired physical mobility, ineffective airway clearance and impaired parenting. During the caring process, the author provided early respiratory training, mental support and holistic and individual nursing care; using team cooperation to propose rehabilitation plan and help the patient recover her maximum function in the affected limbs and weaning ventilator support successfully; providing information regarding care of her premature infant in order to facilitate building positive parenting relationship. In the following-up after discharge, the patient has improved her ability in self-care and adapt to the role of motherhood. The article provides a guideline in providing effective care for other similar patients in the future.
