護理一位精神分裂症合併酒精依賴共病患者酒癮復發之經驗
Nursing Care Experience of a Relapsed Alcohol Dependent Patient with Co-morbid of Schizophrenia
摘要
本文旨在呈現護理一位有精神症狀干擾及酒癮復發的精神分裂症合併酒精依賴患者之經驗，於民國97年3月15日至4月14日患者住院期間，筆者對患者的身體、情緒、智能、社會及靈性等五大層面進行護理評估，判斷存有護理問題如下：潛在危險性傷害、睡眠型態紊亂、焦慮、感覺及知覺改變。護理過程中筆者藉著與患者建立護病關係，每日監測戒斷症狀變化，增加患者有關酒精戒斷及營養失衡之認知，提供改善食慾、補充水分等措施，避免營養不良、戒斷症狀等潛在危險性傷害進一步地發展；鼓勵維持足夠活動量，並提供夜間舒適的就寢環境，進而維持夜間睡眠至少六小時以上，以改善睡眠型態紊亂；引導學習情緒調適與轉移注意力等因應技巧，以改善焦慮與感覺及知覺改變等護理問題，使患者達到內、外在人際適應良好，重新面對精神復健及酒癮戒治等處遇計畫，出院後至97年7月14日為止，據筆者以電話追蹤了解，患者仍能維持精神復健生活與戒癮復原狀態。本文整理此次成功照護一位酒精依賴及精神分裂症共病患者酒癮復發的護理經驗，期待藉提出分享供臨床照護同仁參考，在面對此類酒癮共病患者時能具備更專業的知識、態度與自信。

This article describes the one-month acute nursing care of a schizophrenic patient with dual diagnosis of alcohol dependence from his relapse to recovery. Initial nursing assessments of the patient's health status, emotional control, intelligence, social behavior, and spirit revealed the following nursing problems: potential for injury, sleep disturbance, anxiety, and alteration in sensory and perception disorder. The nursing care activities consisted of monitoring signs and symptoms of alcohol withdrawal, promoting patient safety, hydration, patient teaching on the relationship between alcohol consumption and malnutrition, improvement of nutrition and maintaining physical activity. Providing a comfortable sleeping environment helped the patient maintain a minimum of 6 hours quality night time sleep. The patient's anxiety status and interpersonal relationship improved by learning emotional control and refocus. The patient remained sober and rehabilitated during a 3-month post discharge telephone follow-up.
