照顧一位壓力性損傷病人及家庭照顧者負荷過程之護理故事

壓力性損傷（pressure injury）簡稱壓傷，在數小時就會出現，當未有預防壓力性損傷相關知識來預防及治療時，壓力性損傷就會持續再發生及導致病人反覆入院，這對於家庭照顧者出現自責感，也會帶來負荷及負面的健康影響，本文旨於以家庭為中心協助壓力性損傷病人反覆入院及照顧者負荷超量之護理經驗，探討照顧壓力性損傷個案及家庭照顧者之過程，以卡加利家庭評估使讀者更清楚家庭狀況，使用壓力性損傷指引建議，藉由家庭會議了解家庭照顧者需求及家屬偏好，選出可接受的項目來執行，同時於照護期間在資料庫搜尋有關壓力性損傷照者之影響因子後，將提升照顧者保護因子及降低障礙因子等措施，實際執行在個案身上，適情況而做修正及改善，再評值成效，使個案及照顧者能恢復健康，期許本篇文章能於專業領域或家庭上達到幫助。

Pressure injury will appear within a few hours. Implement it on patients, make corrections and improvements as appropriate, and evaluate the effectiveness of the case and the caregiver.
