護理臨床教師教學能力對新進護理師留任率之影響

The Effect of Clinical Preceptors' Teaching Competence on The Retention Rates of New Graduate Nurses

摘要


新進臨床的護理師面對陌生的工作環境以及臨床上各種突發的狀況,常會產生現實震撼,再加上護理能力與技能不足,導致留任意願低,因此臨床上希望藉由護理臨床教師一對一的指導,能讓新進護理師成功留任。然而目前國內外研究探討護理臨床教師對新進護理師留任之影響仍闕如,故此研究主要目的是探討護理臨床教師教學能力對新進護理師留任率之影響,期能作為未來師資培育計畫之參考。本研究採回溯性調查研究,以北部某醫學中心有輔導新進護理師之護理臨床教師為研究對象,分析護理臨床教師的基本屬性(性別、學歷、單位屬性、進階層級、臨床年資、年齡、專業證照數)與教學能力概況,並探討臨床教師教學能力對新進護理師留任率之影響。研究結果發現外科病房、加護中心及特殊單位(門診、手術室、婦產科病房)之護理臨床教師教學能力得分皆顯著低於內科病房,但護理臨床教師的基本屬性與教學能力均未顯著影響新進護理師的留任率。建議未來可依單位特殊性量身訂做不同之護理臨床教師訓練方式,尤其是加護中心與特殊單位之教師培育,以提升加護中心或特殊單位臨床教師之教學能力。

New nurses face unfamiliar working environment and various clinical emergencies, and their inability to cope may lead to low retention rate. The use of nursing clinical preceptors to pair one-on-one guidance to new nurses may help new nurses successfully transition into their new role and higher retention rate. There are limited research on the correlation between nursing clinical preceptor competence and the retention of new nurses. This is a retrospective study on the impact of clinical nursing preceptors' teaching and the new nurses' retention rate. The data analysis was collected from a medical center's nursing clinical preceptors who were involved in the teaching of new nurse post graduate year (NPGY). The results show that there were no significant differences in the retention rate of new nurses in clinical preceptor's gender, age, education, ward attributes, advancement level, clinical seniority, number of preceptor's professional certificates, and teaching competence. In the evaluation of preceptor's teaching competence, the assessment scores of the preceptors in surgical wards, intensive care wards and special care units (out-patient department, operating room, obstetrics and gynecology ward) were significantly lower than those in the internal medicine wards. Recommendation for future preceptor training programs tailored to the specialty care units, especially emergency room and intensive care unit.
