美國密西根州長期照護機構參訪心得與省思

摘要

此次參訪目的爲提升學生在長期照護領域之興趣及擴展國際觀，特甄選十名學生及二名教師前往美國密西根州進行參訪學習活動。參訪活動因應學生學習需求，乃委託在美國經營長期照護機構有成之企業家楊逸鴻博士規劃行程。參訪時間爲2008年6月14日至22日，此次參訪範圍從健康老人至重度照護需求老人、社區至機構式照護、一般照護至特殊照護需求之機構；參訪機構含括學術機構、政府單位、社區照護機構、老人公寓、照顧住宅及護理之家等單位共二十餘處。參訪密西根州照護機構發現其照護朝向以「家」爲目標設計環境及照護內容，強調老人可保有自主權及隱私，擁有私人居住空間之控制權，並提供個人化的照護。住在社區中老人也可以利用基層醫療機構、日間照護、居家照護及各式照顧服務，如社區送餐、送藥、輔具的提供使老人可獨立生活在家中。從此次的參訪中可了解美國長期照護發展及市場的趨勢，其個人化及多元照護的經驗，可提供在快速老化的台灣社會中長期照護政策及市場發展的參考。

Chang Gung Institute of Technology had sent ten students and two faculties to visit the long term care facilities in Michigan. The purposes of the visit were to promote the students' interests to long term care and to broaden their global prospective. The learning abroad itinerary was arranged by Dr. Theresa Chang who has been a very successful entrepreneur and three long term care facilities owner in the United States. The dates of visiting were from June 14 to 22, 2008. The scope of visiting ranged from the care of healthy elder to intensive care needs elder, from community-based care to institutional care, and from general care to special care. Among more than 20 institutions we visited including universities, government agencies, community health care agencies, senior apartments, assisted living facility and nursing homes...etc. We found「home-like」is the whole mark of the care, including the environmental design and the contents of the care. They emphasize autonomy, privacy, controlling of personal living space, and individualized care. The senior citizens who live in the community can utilize basic medical facility, day care center, and other care services such as meal on wheels, drugs home delivery, and assistive device. So they can live at home independently as long as possible. After the trip, we had better understanding the development of long term care and marketing trends in the United States. Their individualized and diversity of care may serve as the references for Taiwan in the long term care policy making and the development of marketing.
