Spontaneous Renal Rupture Owing to A Ureteral Stone: A Case Report
因輸尿管結石引發自發性腎盂破裂：病例報告
摘要
腎盂發生突發性破裂是一個非常少見的疾病，根據研究有62%自發性腎盂破裂肇因於腫瘤，其餘則因血管變異及嚴重感染，正常腎盂發生自發性破裂實為罕見。並且幾乎所有病例皆好發在右腎。而診斷需依靠腹部超音波檢查及電腦斷層以能提供診斷依據。治療方式可以經皮穿刺腎臟造廔或是輸尿管鏡及雙J導管置入作為減壓方式，特別在因輸尿管阻塞造成的腎盂壓力驟增，減壓引流為一較好治療方式。本院提供一45歲女性病患，因右側突發腹痛求診，腹部超音波及含顯影劑之腹部電腦斷層發現右側近段輸尿管結石合併右腎外側積水及顯影劑外流，證實為自發性腎盂破裂，經輸尿管鏡及雙J導管置入減壓後，病患安然返家。因輸尿管結石引發自發性腎盂破裂主重及早診斷，再此提出此一個案報告，以作為參考。

Spontaneous renal rupture is a rare event that may occur into the pelvicalceal system, the perirenal space, or the peritoneal cavity. This was a 45 years old female patient who had a history hypertension combined anemia. Sudden onset of right flank attacked the patient without any trauma. Abdominal computed tomography (CT) scan with contrast showed a ureteral stone over right side upper ureter. Dilatation of proximal ureter and a cystic lesion over lower pole of right side renal fornix with urine extravasation were observed as well. Owing to the high mortality, emergent surgical repair or decompression were necessary. We performed urethrocystoscope with double J catheter decompression . The patient was alive after the emergent surgery. Early diagnosis and surgical intervention with nutritional support was effective.
