有氧運動對阿黴素抗癌療效及其所引發心臟毒性之影響

Effects of aerobic exercise intervention on the anticancer efficacy of doxorubicin and on induced cardiotoxicity

摘要


化療藥物阿黴素的副作用會導致心臟衰竭,主要原因是會造成生物體的氧化壓力和心肌發炎,而相關文獻指出有氧運動可以抑制氧化壓力與癌細胞的成長及降低心肌發炎指數與阿黴素所引起的心臟毒性,所以有氧運動或許可以提升阿黴素的抗癌功效,並且可能降低以阿黴素化療而引起的心臟毒性,但到目前為止,確很少有相關的文獻報導。目的:因此本研究主要目的是在探討有氧運動是否可以提升化療功效,並同時減緩阿黴素的心臟毒性。方法:以6-8週齡的小鼠36隻為實驗對象,將其隨機分派為下列四組(一)控制組、(二)腫瘤細胞注射+阿黴素注射+未運動組、(三)腫瘤細胞注射+阿黴素注射化療與同時運動組、(四)腫瘤細胞注射+阿黴素注射化療後再運動組,經四週後,利用心臟超音波分析其心臟功能及腫瘤大小,最後犧牲取血液與心臟組織後,再進行相關生化分析。結果:研究結果顯示有氧運動並沒有明顯提升阿黴素的抗癌功效,雖然運動組的癌細胞比不運動組的體積小1/4,心臟超音波發現化療後左心室射出分率(FS)明顯增加;在生化分析結果方面,有氧運動能減緩小鼠之心臟細胞發炎表現(NF-kB)、氧化壓力表現(SOD2)及細胞凋亡的表現(Cyt-c, BCL-2, BCL-2/BAX)。結論:有氧運動不會影響化療功效,但確可以減緩小鼠因以阿黴素化療而所引起的心臟毒性。

The side effect of chemotherapy drug doxorubicin (DOX) can cause heart failure. The mechanisms of cardiotoxicity are oxidative stress and cell inflammatory responses. Some evidences have showed that aerobic exercise can improve antitumor activities of DOX. Other researchers have found exercise can improve antioxidant and anticancer activities and reduce cell inflammatory responses. Therefore, it is possible that aerobic exercise can improve antitumor activities of DOX and reduce cardiotoxicity of DOX at same time. In this study, we didn't find exercise improves antitumor activities of DOX. However, we did find the average volumes of tumors in exercise groups are 1/4 smaller than sedentary group. In cardiac functions, we didn't find any significant values in echocardiogram. On the other hand, we found exercise did improve anti-inflammatory related protein (NF-kB), antioxidant related protein (SOD2) expression and inhibit cell apoptosis protein (Cyt-c, BCL-2, BCL-2/BAX) expression. These results indicated that aerobic exercise can attenuate DOX induced cardiotoxicity without affecting the efficacy of DOX.
