一位糖尿病足病變行截肢手術病人的護理經驗

摘要

本個案報告係協助一位糖尿病足病變病人面臨截肢手術過程的護理經驗。筆者在民國2002年9月12日至10月2日期間以觀察、會談、傾聽等技巧，並運用Gordon十一項功能性健康型態評估，發現個案於截肢手術前不僅要承受疾病及手術的威脅，術後更要面對身體功能喪失與外觀永久的改變，因此出現焦慮、疼痛、高危險性感染及身體心像紊亂等問題。筆者於護理過程中運用主動關懷、同理等支持方式，藉由直接護理與個案建立良好護病關係，並取得家屬的支持與配合，以專業知識、技能協助個案面對截肢的適應與恢復健康。

The purpose of this article is to present the nursing care experience of patient suffering diabetic foot and facing the amputation surgery. From September 12 to October 2. 2002, the author adopted the techniques of observation, conversation, audience etc and used the criterion of Gordon to estimate the functional healthy of the patient. The results showed not only the threatening of disease and stress of surgery, but also the functional loss of limbs and permanent change of the appearance of the patient to confront after operation. Therefore, anxiety, pain, high risk for infection and body image disturbance deviated problems may be existed. During nursing process, the author adopted the modalities of support, such as active assiduity and sympathy to establish a good nursing-patient relationship and gain the buttress and coordination of the family. Thus, we can also assist patient to get adaptation of amputation and recovery of health by means of professional knowledge and executions.
