以方塊踏步團體活動介入於日照中心長者衰弱改善的效果:一項前驅研究

The effect of Square-Stepping Exercise Group for Elderly on frailty improvement in Day Care Center: A Polit Study

摘要


新冠疫情需兼顧防疫社交距離,長者活動課程受限,致衰弱問題加劇。透過老人週全評估及實證建議,找出最適合失智且衰弱的長者兼顧防疫規定下最佳衰弱改善研究方案。以日照中心長者為對象進行方塊踏步運動訓練,每週一次,每次一小時,共九週,介入前後評估SOF衰弱量表、五次椅子坐站(下肢功能)、計時起走、四階段平衡檢測,於九週結束後進行後測,本研究採用SPSS26.0軟體進行分析,以描述性統計、相依樣本t檢定、卡方檢定、Wilcoxon符號等級檢定進行分析。活動後衰弱期長者可從57%降至14%,而5次椅子坐站測驗可從前測22.29±9.43秒,進步到後測12.45±2.63,並呈現統計顯著水準(P=0.028),顯見活動成效。此運動可改善下肢生理機能延緩衰弱,建議未來研究可在開發更大量樣本,以驗證研究多面向成果,此方案可作為日照機構做為規劃高齡者健身活動的另一選擇之參考。

Due to the COVID-19 pandemic, social distancing needs to be taken into consideration, and activities and courses for the elderly are restricted, which intensifies the debilitating problem. Through comprehensive evaluation of the elderly and empirical suggestions, we will find out the best research plan for improving the frailty that is most suitable for the elderly with dementia and frailty taking into account the epidemic prevention regulations. The elders in the Day Care Center were subjected to square stepping exercise training, once a week, for one hour each time, for a total of nine weeks. The SOF frailty scale, five times of chair sitting and standing (lower extremity function), timed walking, and four-stage balance were evaluated before and after intervention. The post-test was conducted after nine weeks. This study used SPSS 26.0 software for analysis, and used descriptive statistics, dependent sample t test, chi-square test, and Wilcoxon sign rank test for analysis. The elderly in the debilitating period after the activity can be reduced from 57% to 14%, and the 5-time chair sit-stand test can be improved from 22.29±9.43 seconds in the pretest to 12.45±2.63 in the posttest, and it is statistically significant (P=0.028), showing the effect of the activity. This exercise can improve the physiological function of the lower limbs and delay the frailty. It is suggested that a larger number of samples can be developed in future research to verify the multi-faceted results of the research. This program can be used as an emerging option for fitness activities for the elderly in sunshine institutions.
