摘要

全球人口快速老化，世界衛生組織以及許多國家均以「活躍老化」的概念提出策略，以使長者在老化的過程中能提昇生活品質，並獲得最大的健康、社會參與和安全的機會。雖然老年人有相對較高的醫療服務需求，但健康照護機構對於老年人的友善服務仍有待改善。2010年由當時國民健康局（2013年升格為國民健康署）開始推動「高齡友善健康照護機構認證」，該認證包含「管理政策」、「溝通與服務」、「照護流程」、「物理環境」等四大標準含60項條文，提供導入架構，以推動各機構提供高齡友善的服務，臺灣也成為國際間第一個由政府全面推動該類認證的國家。高齡友善照護機構可透過推廣高齡友善服務，營造對長者友善的環境和服務，以滿足長者不同疾病型態和健康照護過程中的需求；也應以「全人健康」為取向，參與跨領域資源整合，提供高齡者連續性的全人照顧，讓每一位長者都能享受有品質、有尊嚴的健康老化與善終。

The global population is aging rapidly. The World Health Organization and many countries have been working with great vitality to enhance the quality of life and to create more opportunities of improving health, social participation and safety for the elderly based on the concept of ”active ageing”. The fact that older adults have relatively greater needs of health care than the general population renders the need to provide age-friendly services an issue of crucial importance to healthcare institutions. Since 2010, the Bureau of Health Promotion has launched the accreditation of ”Age-Friendly Health Services,” which establishes standards for the implementation of a framework with 60 items in four major categories, including ”management policy,” ”communication and services,” ”care process” and ”physical environment.” Taiwan is the first place where such an accreditation is nationally promoted by the government. The health services should improve their quality of service by implementing the Framework of Age-Friendly Health Services” to establish an environment meeting the needs of the older adults with various health conditions. Hospitals and allied healthcare institutions should further join force to expedite cross-disciplinary resource integration for ongoing promotion of holistic elder care and active aging.
