Boerhaave's Syndrome after Forceful Swallowing
強烈吞嚥後造成食道自發性破裂
摘要
Boerhaave's症候群，又稱食道自發性破裂。臨床病例少見，但死亡率高。一般多見在中年男性病患，並常由劇烈嘔吐引發。我們報告一案例，一個年輕男生，在用力吞嚥食物後，喉部、頸部及胸部開始疼痛。到急診檢查後發現，病患是食道自發性破裂。病患住院七天後，治癒而離院。對食道自發性破裂之病情預後，早期診斷及完整的檢查是關鍵。

Boerhaave's syndrome (spontaneous esophageal rupture) appears most commonly in males between the ages of 40 and 60. Severe vomiting followed by excruciating chest pain are the classic clinical signs, often in conjunction with a history of over indulgence in food and alcohol. We report a rare case of spontaneous rupture of the esophagus in a boy who visited our hospital with difficulty swallowing and neck pain after forcefully swallowing some food. This case was successfully treated by conservative therapy. The absence of vomiting and chest pain prior to presentation is the distinguishing feature of this rare case.
