口腔健康狀況與全身系統疾病相關性之探討

摘要

口腔健康是身體健康之基礎，然而全身健康狀況也有可能影響口腔之健康情形。本研究之目的在瞭解口腔健康與全身系統疾病之相關性。研究對象為民國九十年十二月到民國九十一年六月間，在高雄市某市立醫院作全身健康檢查之民眾，在調查期間共有177位患者接受健康檢查及接受口腔健康檢查。藉由口腔檢查、全身檢查報告並利用SPSS等統計分析了解口腔健康與全身系統的相關性。
研究結果顯示性別、年齡與全身健康有關，男性總膽紅數和尿酸異常的比率高於女性。飯後血糖與尿素氮之異常比例，有隨著年齡增加而增加的現象。在口腔健康方面，發現隨著年齡增加膺復現況、牙周附連喪失及缺牙等情形則有越來越嚴重現象。口腔狀況與全身健康情形，飯前血糖異常者其牙周狀況較差，囊袋較深牙周附連喪失也較嚴重。而總膽固醇異常者牙周狀況也較差。三酸甘油酯異常者其顳顎關節異常的比率較高。口腔黏膜異常者則肺功能低。在C肝異常者有較高的缺牙數，自覺全身狀況有異常則缺牙數有偏高的現象。
本研究發現身體與口腔狀況有多項顯著相關，顯示口腔與全身狀況有其因果關係，從本研究之結果希望能提醒民眾口腔疾病與身體健康狀況會相互影響。並建議醫療單位能夠重視，將口腔狀況納入健檢項目，提昇口腔保健觀念。

Oral health is the basis of general health condition and it might affect oral health. The purposes of this study were to understand the relations between oral health and general health conditions. The 177 study samples were selected from people who came for a general health examination in one of the Municipal Hospitals in Kaohsiung from Dec. 2001 to Jun. 2002. The relations between oral health and general health conditions were analyzed with SPSS statistic utility.
The results showed there were correlation between general health condition and sex as well as age. The abnormal ratio of total bilirubin and uric acid were higher in male than female. The proportions of high blood sugar level before meal and abnormal BUN value increased as age increased. The data also showed ratio of poor prosthetics was increased; attachment level and missing teeth were getting worse as age increased. As for the relation between oral conditions and general health, periodontal disease was severe in the abnormal before meal blood sugar level group as well as in the abnormal cholesterol group; attachment level was also severely loss in the group with deep periodontal pocket. The proportion of TMj disorder was increased in the abnormal TG group. The mean number of missing teeth was higher in the abnormal hepatitis C group, the mean missing teeth was also high in the group with self-awareness of disease.
There were significant relation between oral conditions and general health conditions in some measure. From the results of this study, it might remind the public of the important of the oral health as well as the general health condition.
