提升手術病人安全維護流程正確率於手術全期護理之改善方案
Effectiveness on Integrity of Patient Safety during Preoperative Nursing
摘要
「病人辨識」是病人安全中極重要的層面，是執行手術全期護理的一個重要課題。本專案由95年6月至12月實施，爲執行正確的病人辨識及交班流程，杜絕醫療程序之病人辨識錯誤事件，以增加病人到醫療院所求診的信任感及安全感。經分析發現手術全期中之病人辨識錯誤因素有：醫護人員未確實執行（詢問病人姓名、手術部位註記、配戴手圈）及交班內容不完整等因素。經由教育訓練、製作交班內容標準書、定期查核等措施，改善病人辨識正確率由89.26%至97.53%；手術前準備項目、手術中及手術後交班內容完整率由93.91%至98.25%，透過本專案單位同仁更瞭解手術病患安全照護的重要性。

Patient identification is one of the most important issues in perioperative nursing. This project was conducted from June to Dec. 2006 in single community teaching hospital. In order to prevent the misidentification, correction of patient identification and handover procedure is needed. We found that the possible causes of misidentification are incompletely implements (identify, marking, wristbands) and nurse shift reporting. After revising the six-month learning lesson, we completed a perioperative guideline and established a checklist. It improves the patient identification rate from 89.26% to 97.53%, the inoperative and postoperative handover rate from 93.91% to 98.25%. We recommended this project could fulfill the patient safety during perioperative nursing.
