加速規應用於功能性前伸測試信效度研究

The Reliability and Validity of the Tri-axial Accelerometer Using in the Functional Reach Test

摘要


目的:透過佩戴在第七頸椎(cervical vertebra 7,簡稱C7)、第三腰椎(lumber vertebra 3,簡稱L3)與外踝上方十公分(10 centimetersabove extramalleolus,簡稱EM)的三軸加速規,量測高齡者功能性前伸測試,以釐清加速規應用上的信度、效度及最佳佩戴位置。方法:本研究徵求健康高齡者共31位,參與功能性前伸測試共兩次,兩次各間隔一天以上,三軸加速規佩戴於C7、L3與EM,並將輸出的總向量換算成標準化的路徑長度(normalized path length,簡稱NLP)、峰峰值(peak-to-peak,簡稱p2p)、平均方根(root mean square,簡稱RMS)、曲線下面積(area under the curve,簡稱AUC),比較四種參數內組相關係數 (intraclass Correlation Coefficient, ICC),以及四參數與功能性前伸測試成績的相關性。結果:三軸加速規在功能性前伸測試的再測信度,以佩戴在L3的ICC在NPL與p2p兩參數數值為佳,其他則皆為優良,並皆達到顯著相關;與校標相關程度則L3佩帶位置有p2p及RMS與AUC有達高度相關(.63-.64),C7佩帶位置有p2p及RMS與AUC有達中度相關(.40-.44),EM佩帶位置有RMS與AUC有達中度相關(.63-.64)而p2p則達低度相關(.27)。結論:高齡者的功能性前伸測試,三軸加速規最佳佩帶位置是L3,建議採用p2p、RMS、AUC的資料,提升平衡能力之評估信效度。

Purpose: To establish the reliability and validity in the three located tri-axial accelerometer during elderly Functional Reach Test. Methods: Recruited 31 health elderly to join this study. Elderly were asked to performance the Functional Reach Test for two times intervals more than one day. Tri-axial accelerometers were attached at cervical vertebra 7 (C7), lumber vertebra 3 (L3) and 10 centimeters above extramalleolus (EM). The acceleration was calculated form magnitude to normalized path length (NLP), peak-to-peak (p2p), root mean square (RMS) and area under the curve (AUC). The intraclass correlation coefficient (ICC) and 95% confidence intervals were used to determine the ratio of between-test for the Functional Reach Test. The correlation between four parameters and Functional Reach Test distance were calculated by Pearson correlation coefficient. Result: The test-retest reliability in most parameters of the three locations were excellent (ICC ≧ 0.74).The NPL of the L3 and the p2p of the EM were good (0.74 ≧ ICC ≧ 0.60). However, the only the parameters of p2p, RMS and AUC in L3 had moderate correlation with Functional Reach Test scores (.63- .64). Conclusion: L3 is the best location for tri-axial accelerometer during Functional Reach Test. The appropriate parameters during the Functional Reach Test in elderly are p2p, RMS and AUC.
