食道癌末期病人臨終照護之護理經驗

Nursing Experience with Terminal Care of a Patient with End-stage Esophageal Cancer

摘要


本文敘述一位食道癌末期病人,因病情突然惡化住院進入加護病房治療的護理經驗。個案於96年6月10日至96年7月2日的照護期間,筆者運用Gordon十一項功能性健康評估,發現個案健康問題有:呼吸道清除功能障礙/與呼吸道分泌物多稠及無效性咳嗽有關;焦慮/對於遵從醫療行為仍出現生理情況惡化造成沮喪有關;疼痛/因癌症引起有關;預期性哀傷/與面對死亡有關等健康問題。護理過程中筆者運用觀察、身體評估、會談等方式與個案建立良好護病信任關係,並依安寧療護理念,以同理心、陪伴、傾聽、主動關懷等方式,提升個案正向態度、減少個案及家屬分離焦慮,獲得心靈安適,以克服疾病過程變化及死亡恐懼感,並協助個案家屬面對死亡,陪伴個案人生最後旅程,希望透過此篇文章與同仁分享此臨終照護經驗。

This article describes the nursing experience with a rapidly deteriorating end-stage esophageal cancer patient at the intensive care unit. The duration of nursing care was between June 10 and July 2, 2007. Based on Gordon's functional health assessment, the patient's health problems included inactive elimination of the respiratory passages, anxiety, chronic pain, and anticipatory grief. The patient was given individualized and active care during the nursing process. A good nurse-patient relationship was established by keeping the patient's positive attitude, reducing the patient's and his family's anxiety, giving him peace of mind, helping him adapt to changes in the disease process, helping the patient and his family face death, and accompanying the patient until the last moments of life. Care was based on the concepts of hospice care including empathy, companionship, and listening. The methods of data collection included observation, physical evaluation, and personal interview. The objective of this case report was to share the nursing experience with clinical nursing staff caring for terminal patients.
