摘要

台灣已將邁入高齡社會，隨著人口持續老化，我國失能人口持續增加，對長期照顧之需求也隨之成長。在各類長期照顧服務模式中，居家服務是最基礎的骨幹服務，其提供失能者的身體照顧、家事服務、和健康相關服務，是所有失能者執行日常生活所需的支持性服務，經由到宅的居家服務，讓需要服務者可以居住在原本的地方，是落實在地老化照顧理念不可或缺的服務。在正式部門所提供的照顧中，居家服務在設計上為補充式服務，居家服務的介入影響了失能者的照顧。而居家服務方案的發展因政策上之改變，其服務費從免費到部分負擔的歷程對服務使用是否造成影響？故本研究著眼於在居家服務發展的歷程中，居家服務方案的轉變如何影響照顧安排，透過訪談暸解服務使用者服務使用之變化，歸納影響服務使用之原因，並進一步評估目前的給付時數與部分負擔比例之適當性，提供未來規畫長照保險之參考。

Because of the floridisation in Taiwan, the population who are disable continue to increase and the demand of long term care increase, too. Home care service is the basic service in all kinds of long term care, which including body care, house keeping, and health service. Home care service provide daily service for disability, let them live in their house. It is the indispensable service for aging in place. Home care service is designed to be supplementary service in formal care system. It influences the care demand of disability. The home care program develop with the care policy, and the charge of home care service is from ”free” to ”part pay by user”, does it influence the service use? The research emphasize that home care service develop process, which influence the arrangement of care. Using interview to find out the difference of service use, and appraise the hours and the part pay ratio. Further, the result will be a consultation for long term care insurance.
