足球選手膝關節前十字韌帶重建手術之復健研究─以屏東教育大學男子足球隊守門員江勇玄為例

摘要


由於守門員的前十字韌帶斷裂是很少見的,為了預防這類似的情形再度發生,本研究針對一位足球守門員其十字韌帶斷裂發生原因、肌力復原情形和各項復健計劃之執行及成效探討來做分析,希望可以提供給未來足球員運動傷害時的參考,本研究對象為一名屏東教育大學足球守門員其年齡為24歲,身高175公分,體重85公斤,在一次救球當中發生前十字韌帶斷裂,本研究將以紙筆及錄音機紀錄的方式針對其發生之原因和各項復健計劃來做訪問,訪問對象包含選手及復健師另外也有復健前後施予Kim-Com等速肌力測試來觀察其肌力復原情形研究所得結果如下:一、前十字韌帶斷裂的原因為比賽中為了要擋球,左右位移側併步加上轉身,經由醫師檢驗發現前十字韌帶斷裂、內側副韌帶撕裂、半月軟骨破損。二、肌力是由等速肌力測試儀器所測試出來的,由等速肌力測試所得的數據,是取等速肌力評估表裡面的最大力量和最大力矩做為復健前與復健後之比較,證明復健後比復健前的肌力較為進步。三、根據醫師指示在術後初期,做一些簡單的負重訓練,以增加下肢肌肉強度,中期可針對大腿肌和小腿肌做適量復健,復健後期則強調訓練全身肌肉的協調性,另外強度比較大的動作,則建議於復健完全結束後在進行。

As the prevalence of anterior cruciate ligament rupture of goal keeper is rare. In order to prevent the recurrence of similar situations, the causes, muscle strength recovery of the rehabilitation program, implementation and effectiveness of the anterior cruciate ligament rupture were analyzed In this study. The subject is a a goal keeper of soccer team in Pingtung university of education, his age:24 yrs, height: 175 cm, weight:85 kg. The injury occurred in a diving process. The pen, paper and a tape recorder were used to access the data of occurrence and rehabilitation plans. The rehabilitation staff's opinion and muscle strength evaluated from Cybex isokinetic dynamometer were also collected. The results were showed as below:1. Anterior cruciate ligament rupture is due to competition in order to block the ball, step with left displacement of the side. After the physician examinations, results showed that the anterior cruciate ligament rupture, medial collateral ligament tear, meniscus damage.2. The data of Isokinetic assessment form Cybex is the maximum power and maximum torque for comparison of before and after rehabilitation, the results showed that there was improvement in muscle strength after rehabilitation.3. According to physicians' advice, during early stage, some simple weight training may be benefit to increase lower limb muscle strength. In medium-term, the large amount of leg and calf muscle training may be taken.In post-rehabilitation, training stressed the coordination of muscles. The greater strengthen training should be taken in the end rehabilitation or almost completely recovery.
