健康照護矩陣應用於跨領域照護團隊運作之經驗

Experience of application of Healthcare Matrix to Transdisciplinary Teamwork in Patient Care

摘要


跨領域照護團隊是由不同專業背景者組合而成,團隊成員擁有各自的知識及專業,透過團隊的協調、溝通、合作及整合,促使病人得以獲得最佳的醫療照護。健康照護矩陣 (Healthcare Matrix) 是一種以稱職能力來評估醫療品質的工具,其能夠全面性的評量病人所接受的照護,找出團隊溝通、臨床記錄、病人安全與工作效率等問題點。醫療照護團隊成員可於團隊會議前透過矩陣表單的填寫發現臨床照護問題,並且在會議上分享各專業領域人員對於病人照護工作經驗。本研究主要是探討將健康照護矩陣 (Healthcare Matrix) 導入跨領域照護團隊運作,是否讓本院跨領域團隊成員能更深入理解跨領域團隊運作模式,並且讓各領域人員能對矩陣裡的各項要素有系統性及全面性的學習,透過專業溝通技巧以知識、技能、態度來達成共同的目標,以避免錯誤發生。研究結果發現:1.健康照護矩陣的導入可以促使各專業領域的人員對於了解並執行自我專業有更高的期許;2.在人際溝通的技巧提升,可以讓團隊成員更加能夠勇於表達出自己想要表達的內容;3.健康照護矩陣的導入除了可以認識自己之外,亦可以透過各專業間的討論,進而去檢討自己所不足的地方,藉以提升了各自領域間的專業素養,提供給病人最適切的醫療品質及持續性的照護計畫。

A transdisciplinary team, consisting of members from various disciplines and professions, provides patients with the best quality of health care, through collaboration and communication among team members. Healthcare Matrix formulation, a practical tool for assessing health care quality, can comprehensively evaluate the health care received by patients. In addition, this formulation helps to find out where the problems are, in the aspect of teamwork communication, clinical records, patient safety, work efficiency, and so on. The team members can fill out Healthcare Matrix forms and share their experience of taking care of patients during the transdisciplinary team meeting. This study was to investigate whether application of Healthcare Matrix to transdisciplinary teamwork allows team members in our hospital to profoundly realize the model of transdisciplinary teamwork, and allows the members of various professions to have more systematic and comprehensive learning about each element of Healthcare Matrix, through professional communication to achieve the team goals and avoiding mistakes. The results showed that application of Healthcare Matrix to transdisciplinary teamwork helped team members (1) to have a better understanding of their own professional knowledge; (2) to improve their communication skill, allowing them to express themselves more clearly; (3) to know themselves better and review their own insufficiencies through interprofessional discussions, which can increase their own professional knowledge and offer patients the most suitable healthcare quality and persistent care plan.
