積極血糖控制課程對重症加護人員認知、態度與行為影響之前驅研究

Knowledge, Attitude and Behavior of Critical Intensive Nurses After a Pilot Study of Tight Glucose Control Education

摘要


背景:重症高血糖容易增加院內感染的發生率、延長加護病房住院天數,以及增加死亡率。目前積極血糖控制課程尚未受到重視,且影響護理人員的認知、態度與行為亦亟待瞭解。目的:針對加護病房人員進行積極血糖控制的教育訓練課程,瞭解課程介入前後認知、態度及行為的差異。方法:為前驅研究,採前實驗設計法,以立意取樣方式,選取東部某區域教學醫院20位加護病房護理人員為研究對象。結果:積極血糖控制教育課程介入前後為:認知平均得分,介入前為1.49±0.13分,介入後為1.61±0.15分(t=-3.597, p<.01)。而態度平均得分,介入前為3.80±0.33分,介入後為3.99±0.28分(t=-2.229, p<.05)。最後行為平均得分,介入前為3.50±0.56分,介入後為3.82±0.21分(t=-2.492, p<.05),三者得分於介入後皆明顯提升。結論/實務應用:接受積極血糖控制課程前、後,加護病房護理人員無論在認知、態度及行為方面皆有提升,期望將此相關課程規劃為臨床在職教育單元內,提升重症病患的照護品質,並降低高血糖所造成的危害。

Background: High blood glucose concentrations may increase the incidence of nosocomial infections, length of stay in the intensive care unit (ICU), and mortality rate of critically ill patients. Education programs on tight glucose control have not been emphasized in the ICU, so the knowledge of intensive ICU nursing staff needs to be investigated. Purpose: The study purpose was to design glucose control education programs for ICU nurses to improve their knowledge, attitude and behavior about blood glucose control. Methods: This was a pilot study with a pre-experimental design method and purposive sampling. A total of 20 intensive care unit nurses were enrolled with inclusion criteria in a regional teaching hospital in eastern Taiwan. Results: The results showed that the ICU nurses had a higher average score of 1.61±0.15 points in knowledge after the curriculum intervention compared with 1.49±0.13 points before the intervention (t =-3.597, p<.01). The average score for attitude was 3.99 ± 0.28 points after the intervention compared with 3.80±0.33 points before the intervention (t=-2.229, p<.05). In the behavior dimension, the ICU nurses had an average score of 3.82±0.21 points post test (pre-test 3.50±0.56, t=-2.492, p<.05). Conclusion: The knowledge, attitude, and behavior of ICU nurses improved after glucose control program intervention. We hope this curriculum intervention can be used in clinical nursing education to increase ICU nurses' awareness of this important issue and enhance the quality of nursing care of critically ill patients.
