Development and Verification of Entrustable Professional Activities (EPAs) for Clinical Training of Newly Recruited Nurses

摘要


背景:可信賴的專業活動(EPAs)被認為是專業工作中不可缺少的常規業務。EPAs是臨床監督者依學員勝任能力,逐步授權進行臨床工作,是基於臨床能力表現所呈現的結果。目的:為新進護理人員的培訓,發展一套可信賴專業活動(EPAs)並檢定信效度。方法:採描述性研究設計,研究團隊以名義團體法和問卷調查,進行可信賴專業活動(EPAs)發展。透過120位護理專家完成EPAs主題確認及建構EPA內容,邀請EPAs專家以EPAs品質評量工具EQual(Queen's EPA Quality Rubric, EQual),評估EPAs結構及品質。再以評分者間一致性檢測其信度。結果:護理臨床專家按重要、常見、危險性,選定培訓期新進護理師的可信賴專業活動共8項。內容效度以EQual評量工具評分皆達4.07分以上。共12位臨床教師參與評分者訓練,評分者間的一致性:6項EPAs達≥ 0.80,達一致性可靠;2項EPAs為0.80 > α ≥ 0.66,一致性尚可接受。結論:建構培訓期新進護理師的可信賴專業活動(EPAs)內容,呈現良好的內容效度及評分者間信度。未來將全面推廣至臨床應用,以進一步驗證其可行性及適用性。

Background: Entrustable professional activities (EPAs) are considered an indispensable routine for professional operation. EPAs are results presented based on the performance of clinical ability, wherein clinical supervisors gradually authorize trainees to conduct clinical work according to their competence. Purpose: Develop a set of EPAs to verify the reliability and validity of the training of newly recruited nurses. Methods: A descriptive research design is adopted. The research team develops the EPAs using the nominal group technique and questionnaire survey. 120 nursing experts verify the EPA subject matters and build the EPA content. EPA experts have been invited to evaluate the structure and quality of EPA using the EPA quality evaluation tool, EQual (Queen's EPA Quality Rubric, EQual). The inter-rater concordance is then verified for reliability. Results: Clinical nursing experts select eight EPAs for the training of newly recruited nurses based on importance, commonness, and risk. The EQual evaluation tool shows a score of more than 4.07 for content validity. A total of 12 clinical teachers participate in the rating training and the inter-rater concordance: Six EPAs score ≥ 0.80, meaning that the concordance is reliable; two EPAs score 0.80 > α ≥ 0.66, meaning that the concordance is acceptable. Conclusions: The entrustable professional activities (EPAs) built for training newly recruited nurses shows good content validity and inter-rater reliability. In the future, they will be fully implemented in clinical application to verify their feasibility and applicability further.
