Unusual Lymphedema: A Case of Lymphoma

A 64-year-old male had diffuse large B-cell lymphoma of the right testis and received right orchiectomy (2009), and then chemotherapy and radiotherapy and achieved complete remission. Recently, he complained of redness and skin rashes with left lower extremity swelling for 2 weeks. Cellulitis was suspected, but treatment with antibiotics had no effect. Sonogram showed subcutaneous edema without deep vein thrombosis. Tc-99m phytate lymphoscintigraphy showed dermal backflow, and lymphedema of the left lower leg. [18F]-fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT) showed left external iliac and inguinal lymph lymphadenopathy, and multiple subcutaneous lesions in the left lower leg. Skin excisional biopsy showed diffuse large B-cell lymphoma. The skin lesions and lymphedema subsided after chemotherapy.
