台灣專科護理師執業自信及相關因素之探討

背景:台灣專科護理師(專師)之角色發展已逾20年,卻少有研究探討其執業自信,回答此研究問題將有助於該角色功能的永續發展。目的:旨在探討台灣專師執業自信程度及相關因素。方法:橫斷面研究設計,以專師為研究對象,採方便取樣,於中區某醫學中心進行收案,以自填式問卷進行資料收集,內容包括專師四大執業能力項目執行頻率及執業自信。結果:共納入107位專師,以女性、具有大學學歷、已婚居多。整體及四大執業項目直接健康照護、教學、照護協調及照護品質監測之自信程度分別為(0-4分)2.24、2.78、1.92、2.59及1.68,相關因素包括:教育程度、專師進階分級、服務科別、年齡、取得專師證照後年資、工作滿意度以及執行頻率,其中教育程度、總執行頻率與工作滿意度具有顯著預測性。結論/實務應用:專師執業自信總平均達中度以上,其中教育及照護品質監測兩面向較低,建議決策單位全面提昇專師教育層級,並增加教育及照護品質監測面向的執業機會及在職訓練。然而,此結果推論受限於橫斷面設計及單一機構樣本。

Background: Although the role of nurse practitioner (NP) has been developed for more than two decades in Taiwan, there were few studies investigating what was the level of NPs' practice confidence. Answers to this question will help the sustainable development of the NP's role function. Purpose: The purpose of this study was to explore Taiwan NPs' practice confidence and its related factors. Methods: A cross-sectional survey study was conducted in a medical center in central Taiwan by using purposive sampling. Data were collected by using a self-reported questionnaire, including questions on four main areas of practice competencies. The NPs were asked to indicated their practice frequencies and confidence levels on each of the question. Results: One-hundred and seven NPs were recruited. The majority were married females who own college degrees. The levels of confidence on the four areas of competency were 2.24, 2.78, 1.92, 2.59, and 1.68 for the patient direct care, teaching, practice collaboration, and quality monitoring respectively. Practice confidence significantly related with educational level, NP clinical ladder level, work unit, age, seniority of being NP, job satisfaction and practice frequency. The significant predicative factors were the education level, practice frequency, and job satisfaction. Conclusions / Implications for Practice: The NPs' overall practice confidence reaches above middle level, but the levels confidence in teaching, and quality monitoring were lower than in the other areas. The policy-making bodies are advised to raise the level of NPs' education, and to promote practices opportunities and continue education on the areas of teaching and quality monitoring. However, the inference of the study results is limited by the cross-sectional design and recruiting sample from a single hospital.
