護理人員職場自尊與留任意願之相關性探討

The Exploration of the Relationship between Organization-based Self-esteem and Intention to Stay in Hospital Nurses

摘要


本研究旨在探討台灣臨床護理人員職場自尊與留任意願之情形、職場自尊與留任意願間的相關性及其相關影響因素。研究採橫斷式相關性研究設計,以臨床護理人員為對象,於北部某醫學中心將符合收案條件之個案皆進行收案。經醫院之IRB 審核通過並取得院方同意後,於101年8月1日起至3月14日進行問卷調查,共發出841份,無效問卷34份,納入分析之有效問卷共791份,有效問卷回收率94.05%。 本研究工具為結構式問卷,包括「個人基本資料表」、「升遷機會量表」、「社會支持量表」、「工作壓力量表」、「工作滿意度量表」、「職場自尊量表」及「留任意願量表」。以SPSS 20.0 for Windows中文版套裝軟體及Smart PLS 2.0套裝軟體進行資料建檔與統計分析。 研究結果顯示: 1. 護理人員職場自尊程度在中等程度(M=3.44,SD=0.48),介於沒意見和同意之間。 2. 護理人員留任意願在中等程度(M=4.24,SD=0.87),介於不確定和略同意之間。 3. 護理人員職場自尊的影響變數有社會支持及工作滿意度。 4. 護理人員留任意願的影響變數有社會支持、工作壓力、工作滿意度及職場自尊,其中職場自尊對於社會支持及工作滿意度影響留任意願皆具有中介效果。 本研究結果提供行政管理者,瞭解臨床護理人員職場自尊與留任意願之感受及相關變數,並做為行政者領導及管理之參考,期以能够提升護理人員職場自尊進而提高留任意願。

Workplace promotion, social support, work place stress, job satisfaction, organization-based, and self-esteem are major psychosocial factors of intention to stay among nurses; however, few studies have analyzed the relationship among them. This study aimed to explore the relationship between organization-based self-esteem and intention to stay among nurses and used structural equation model to test related factors affecting the relationship. A cross-sectional survey was employed and a convenience sample was recruited from one medial center in Taiwan. A total of 791 registered nurses without personal history of psychiatric or neurological disorders and they work full-time for at least 3 months in hospital. A total of 791 nurses completed the following self-report questionnaires over a three- month period in 2012: A questionnaire,which included demographic data, questions regarding possibilities of promotion, Questionnaire of social support, Questionnaire of Medical Workers’ Stress, Questionnaire of job satisfaction, Questionnaire of organization-based self-esteem, and Intent to Stay in Nursing scale.The results showed : 1.The degree of organization-based self-esteem (OBSE) among nurses landed at middle Points (M=3.44,SD=0.48), meaning between no comment and agreeing. 2. The degree of intention to stay among nurses landed at middle points (M=4.24,SD=0.87), meaning between being not quite sure and slightly agreeing. 3. Social support and job satisfaction both played as the variants, affecting the degree of OBSE among nurses. 4.Social support、workplace stress、job satisfaction, and OBSE played as the variants, affecting the degree of intention to stay among nurse. Besides, among all the above-mentioned variants, OBSE played as a mediate role; to be more specific, social support affected intention to stay but OBSE played as a mediate role among them. Same, job satisfaction affected intention to stay, but OBSE also played a mediate role among them. This study can provide medical administerators with the current status of OBSE, and intention to stay among clinical nurses, and make them realize the main factors and variants that affect the degrees of OBSE and intention to stay. We hope this study can be used as the reference data for those who are in charge of making administration and management rules in medical units and hospitals to increase the degrees of OBSE and intention to stay among clinical nurses.
