背景與目的:急救技能是急診醫護人員必須具備的重要能力。團隊資源管理對團隊運作成效具重要影響,情境模擬訓練可有效提升臨床照護能力、跨團隊合作與溝通。因此本研究目的為探討結合團隊資源管理和情境模擬之急救復甦訓練對急診團隊之成效。研究方法:本研究以北部某醫學中心之急診醫療團隊成員為訓練對象。以團隊資源管理為架構,參考團隊情境模擬案例設計臨床教案,以「團隊專業表現整合性評估量表」評量團隊成員臨床表現和技能表現及團隊間互助合作。研究期間共舉辦6梯次的情境模擬急救訓練,總共有27位學員參加,住院醫師9位(33.3%),護理師18位(66.7%),參與學員進行二次急救擬真模擬訓練,作前、後測評量得分。本研究以t檢定分析訓練前、後測差異。研究結果:學員在「團隊專業表現整合性評估」、「情境模擬訓練Orientation評價」、「情境模擬訓練課程滿意度」,於接受訓練課程後的得分皆有增加,且達統計上顯著意義(p<0.01)。學員對所設計的訓練課程給予高度肯定與滿意,除了提高學習動機,更增加對急救執行的自信。結論:本研究結果指出團隊資源管理和情境模擬教學運用於急救復甦訓練,不僅提升急救成效與品質,也對急救團隊的合作技巧與合作表現達到良好提升效果。建議可推展至其他單位,促進急救品質的再精進。
Objective: First aid skills are an important ability that emergency medical team must possess. Team resource management has an important impact on team operation effectiveness. Situational simulation training can effectively improve clinical care ability, interprofessional cooperation and communication. Therefore, the purpose of this study is to explore the effectiveness of first aid training combined with team resource management and situational simulation for emergency medical team. Methods: This study was aimed at the members of the emergency medical team of a medical center in the north. Based on team resource management, the situation simulation scenario is designed by clinical case, and the clinical performance and skill performance and the mutual cooperation between teams are evaluated by the "Team Professional Performance Integration Assessment Scale". Total six cascade of first aid situational simulation training was held, total 27 participants, 9 residents (33.3%), 18 nurse (66.7%). Participants attend twice first-aid simulation training. Use t-test for pre-test and post-test analysis. Results: After the training course students in the "Team Professional Performance Integration Assessment", "situational simulation training Orientation evaluation", "situational simulation training course satisfaction" scores have increased and significance (p<0.01). Students are highly positive and satisfied with the training courses designed, in addition to improving their learning motivation and increasing their confidence in the implementation of first aid. Conclusions: The results of this study show that team resource management and situational simulation training are used in first aid resuscitation training, can improve the effectiveness and quality of first aid and cooperation skills and performance of the first aid team to achieve good results. Suggestions can be extended to other units to promote the quality of first aid.
