護理人力有效應用模式之探討

The Study of Effective Allocation of Nursing Manpower

摘要


本研究之目的是建立人力支援庫及支援模式,並分析瞭解實施人力支援庫後病人所獲得護理時數及護理成本之差異。本研究係以2002年度所發之超時加班費計算,於部內增加編制人員16人及部份工時人員的人組成人力支援庫,以支援因婚、喪、事、病、產假所欠缺之護理人力,來解決護理人員超時工作及公休未休之抱怨。唯2003年因新興疾病嚴重急性呼吸症候群(SARS)疫情侵襲全台,以致本研究僅分析2002及2003年度人力較穩定之三月及四月各護理單位由病人分類系統所統計出來之護理時數及護理人員薪資之差異數,來探討人力支援庫之成效,結果發現使用人力支援庫措施後,病人之護理時數增加,以Wilcoxon Test分析有顯著性差異;而人力成本降低,以簡單線性迴歸分析亦具有顯著性差異,且護理人員在2003年度未領加班費。但人力支援庫人員及被支援單位護理人員之滿意度則需作進一步探討。

The main purpose of this study was to develop a fair, objective and reliable manpower support system to resolve the problem of nursing work overloading, and to evaluate the cost-effectiveness of this system. A quasi-experimental method was used. To develop a manpower support system in 2003, the effect of manpower support system was analyzed from nursing hours and nurses cost between 2002 and 2003. The acquired original data from the patients classification system in NIS( Nursing Information System) and nurses cost from HIS( Hospital Information System), were input into computer with the SPSS software installed. Data were analyzed by frequency, percentage, Wilcoxon Test and Linear regression analysis. The results was interfered by the SARS attack in Taiwan from April to July 2003. After implementing manpower support system, the nursing hours in 2003 was increased comparing with these of 2002, and reached significant statistical differences (P< .05) by Wilcoxon Test. The nurses cost in 2003 compare decreased with 2002, and reached significant statistical differences (p< .05) by Linear regression analysis. The implement of manpower support system was effectiveness regarding to the measurement of nursing hours and nurses cost. But the satisfaction of nurses to manpower support pool and receiving support ward are need to evaluate further.
