運用社交技巧訓練於一位重鬱症役男之護理經驗

摘要

憂鬱症發生的頻率在年輕族群中逐漸增加，而在台灣服役，不同於歐美國家的募兵制，易出現身體和心理及憂鬱和適應問題。本文描述一位22歲服役軍人對部隊環境適應不良，併有失眠、無法執行自我照顧、人際互動不良、攻擊行為的重鬱症個案。筆者透過整體性護理評估發現個案有睡眠型態紊亂、自我照顧能力缺失、高危險性暴力行為-朝向他人、社交互動障礙等問題，以支持性護理措施從關係建立、持續不斷的鼓勵、共同參與團體治療及運用社交技巧訓練於護理過程中。在措施執行後發現個案的憂鬱情緒、不良行為模式及社交能力均明顯改善和提昇。個案出院後可規則返診及維持穩定情緒，並至加油站擔任領班職責，因此藉由社交技巧訓練，解決個案的情緒困擾，並改善人際關係以回歸社區生活之適應。

The rate of depression incidence has gradually increased in the young group. In Taiwan, the way of recruitment soldiers is different from European and American countries. It is compulsory in Taiwan and soldiers are easy to suffer from depression. This study is to explore 22 year-old solider who is unable to adapt the army life. He has insomnia and major depression, unable to carry out self- care, and unable to interact with others. Sleeping disturbance, deficit of self-care, attempt to aggressive, unable to interact with others are found. Strategies of establishment of relationship, encouragement, and participating in the group treatment and social skill training are adopted in the study. The result reveals that depression, behaviour and interaction are improved. The case is followed in the out patient department regularly and has stable mood after discharged. He is back to his work. This result suggests that the social skill training is a good strategy for a patient with depression and interaction.
