照顧一位肝癌末期患者之護理經驗

摘要

本篇在探討一位36歲罹患肝癌末期病患在住院期間所面臨的相關護理問題。護理個案期間自95年7月20日至95年8月8日以及返家後持續的電話追蹤，了解個案後續發展。筆者藉由觀察、會談及應用Gordon十一項功能性健康型態進行評估，確立個案有體液容積過量、疼痛、身體心像紊亂及預期性哀傷等問題。在護理過程中，筆者以同理心、尊重的態度與個案及家屬建立信任感及良好的治療性人際關係，提供持續性之護理，協助個案能維持其基本的滿足及緩解身心的不適，並主動提供家屬有關疾病的治療過程及相關注意事項，此個案雖未住進安寧病房，但仍盡量使其在有限的生命中，能夠維持有品質的生活。期望此護理經驗能與臨床護理人員一起分享。

This article addressed the experience of a 36-year-old hepatocellular carcinoma patient in terminal stage as he was confronted with several nursing problems during hospitalization. One of the authors took care of this patient from July 20 to August 8, 2006 and continued to collect illness course information by telephone when he was discharged. The author assessed by observation, interview, and applied Gordon’s 11-item nursing assessment of health functional patterns to detect if the patient had problems of fluid volume excess, pain, body image disturbance, anticipant grieving, and so on. During nursing process the author used empathy and respectful attitude to establish the reliance and therapeutic relationship with the patient and family. Besides providing continuing nursing care to improve his basic satisfaction and relieve physical and psychological discomforts. The author also provided information of related treatment process and concerns of family. This patient did not enter the hospice ward. However, he could live a life of good quality in his limited life. This nursing experience is to be shared with the clinical nurse staff for improvement of care quality.
