本文個案報告係探討一位再次腦中風患者護理經驗,護理期間自2010年10月20日至2010年11月02日,以Gordon十一項健康功能型態為評估工具在照顧個案過程中,運用會談、觀察、病歷查閱及身體評估等方式收集資料,進行評估,確立其主要護理診斷有︰身體活動功能障礙、潛在危險性傷害、自我照顧能力缺失、照顧者角色緊張及無望感。藉由提供個案及家屬完整的照護過程,筆者運用專業知識、技能,適當的提供護理措施與照護技巧指導,並協助案妻社會資源之運用,增加信心、減輕壓力,使病患學習增強自我照顧能力,運用同理心支持照顧者,鼓勵說出內心感受,並教導主要照顧者及家屬照護技巧。讓個案能在接受護理人員身、心復健照顧後,達到日常生活自理,可下床盥洗。藉此篇報告分享護理經驗,提供臨床上護理同仁,照護此類個案的參考。
The purpose of this report was to present the nursing experience of a stroke patient with recurrent cerebral vascular accident (CVA). The duration of caring for this patient was from Oct 20 to Nov 02, 2010. The author used the Gordon's 11 functional items to collect data and asses this patient. Data collection was from Patent's history, observation, interview and physical examination. The major nursing diagnoses were: impaired physical mobility, risk for impaired injury, self-care deficit, caregiver's role strain, and powerlessness. Firstly, by provide patient and families' whole nursing process and professional knowledge, skills, nursing intervention instructions. Secondly, to help patient's wife resources utilization, built selfconfidence, decrease stresses, patient learn to enhance self-care skills. The nurse support caregivers, encourage them speak feelings by empathy, and teach primary caregivers and family care skills. The patient can share caregiver's burden and actively participate in rehabilitation in order to reduce caregiver's and families' burden. Finally, the patient increase self care's ability and, try to walk to toilet. By sharing this care experience, we hope to improve nursing care quality for cerebral vascular accident patients.
