一位剖腹產初產婦於產後初期哺乳之經驗

摘要

本篇是敘述一位28歲剖腹產初產婦，面對產後自我生理、傷口疼痛、乳房充盈不適，以及初為人母面對新生兒照護各方面之經驗感受。護理期間為2011年10月20日到2011年11月5日，筆者自個案產後第一天開始即提供護理至出院，也於產後第九天及第十四天進行家訪，以田野研究法(field method)，於照護期間過程將個案所表達的語言和行為寫成行為過程記錄，以內容分析法歸類分析，結果發現個案的經驗感受有: 一、哺乳意願之搖擺不定；二、努力嚐試學習哺乳技巧與瞭解嬰兒需求；三、逐漸體驗與嬰兒的生活型態同步。其研究結果可提供護理人員在臨床照顧中更了解剖腹產產婦需求，幫助她們成功哺餵母乳。

This case study described the experience of caring a 28-year-old primipara and assisting her with learning of breast-feeding after the caesarean section. The mother faced physiological discomfort such as wound pain and breast engagement and maternal role transition. The duration of nursing care was from October 20 to November 5, 2011. The subject was interviewed and observed in the field of interaction. There were a total of 6 contacts for the subject. Soon after the interview and field observation, the author wrote down the process recordings. The data was analyzed by content analysis. Data were collected through process recording and analyzed with content analysis. Three main areas of the experience were inductively identified: (a) wavering of breast-feeding willing; (b) trying to learn breast-feeding skills and understand of neonatal physiological needs; (c) consisting with the life styles of the baby and the mother. The findings of this study not only can be applied as a reference for nurses when they are taking care of these women receiving caesarean section, but also can help nurses to assist women to success breast feeding.
