Application of Angio-CT System for Treatment of Recurrent Breast Cancer with Superselective DEM-TACE

血管攝影載藥微球栓塞技術結合電腦斷層治療復發性乳癌患者

摘要


乳癌是全世界最常見的女性惡性癌症的其中一種,手術切除是最主要的治療方式,輔助性化學治療的角色在於抑制腫瘤生長、增加手術切除的機會,和減少腫瘤復發與轉移。傳統上,對於局部擴散性的乳癌患者,介入性放射科醫師利用血管攝影技術,提供微侵入性的治療,例如經動脈化學治療(IAC)和經導管動脈化學栓塞治療(TACE)。Angio-CT系統是結合透視機器與電腦斷層,已應用於某些惡性腫瘤的經導管動脈化學栓塞治療。在栓塞治療過程中,能快速進行電腦斷層掃描,比單獨使用血管攝影能提供更精確的腫瘤和血管支配範圍。有適當的掃描設定,電腦斷層影像還可提供血管解剖與其他重要資訊。我們介紹的病例為一位無法接受手術治療的乳癌患者,利用經導管動脈血管攝影結合電腦斷層的技術,接受載藥微球栓塞治療。

Breast cancer is one of the most common malignancy affecting women worldwide. Surgical resection is a mainstay of treatment. The role of adjuvant chemotherapy is to achieve tumor downstaging, to increase the possibility of surgical resection and to reduce tumor recurrence and metastasis. For the patients with locally advanced breast cancer, interventional Radiologists conventionally provide minimally invasive treatment, such as intraarterial chemotherapy (IAC) and transcatheter arterial chemoembolization (TACE), by means of angiography alone. Angio-CT system combines fluoroscopy and CT scan, and has been used in TACE for certain malignancies. During the procedure of TACE, CT scan can be performed efficiently with angio-CT system, and demonstrates more accurate tumor extent and supplying territories of the cannulated vessels than digital subtraction angiography (DSA) alone. With optimal scanning protocol, CT images can also show detailed vascular anatomy and provide important information for the Radiologists. We demonstrate a case of inoperable recurrent breast cancer receiving TACE with drug-eluting microspheres (DEMs) using combination of angiography and intraarterial CT angiogram with angio-CT system.
