X光巡迴車與在院X光攝影之病人滿意度實證研究-以乳房攝影為例

Comparing X-rays Patient Satisfaction between Fixed Unit and Mobile Car: An Empirical Study on Mammography

摘要


目的:根據國民健康署統計顯示2014年有11817名乳癌新增病例,為我國女性好發癌症排名第一位,乳癌在女性癌症死因排名為第四位。顯示乳癌對我國婦女健康影響甚鉅。因台灣乳癌病人好發年齡約在45-64歲之間,衛生署為使婦女乳癌能早期發現早期治療,2010年起將婦女乳癌X光篩檢,下降至45歲。但是根據統計台灣婦女做乳癌篩檢比例不如外國,為增加婦女做乳癌X光篩檢比例,2011年起有6個縣市衛生局提供數位乳房X光巡迴車服務,以深入社區方式推動乳癌篩檢,因此乳房X光巡迴車成為乳癌篩檢的重要工具。本研究想了解受檢者在乳房X光巡迴車與在院內乳房攝影兩者滿意度差異性。方法:採用Parasuram,Zeithaml以及Berry三位學者所提服務品質模式來發展量表,以可靠性、回應性、確實性、關懷性與有形性等五個構面來設計問卷,調查對象為2018年在本院接受乳癌X光篩檢的婦女,乳房X光巡迴車共計有167人受訪,在院內乳房攝影共計有342人受訪。結果:滿意度調查結果在有形性構面及確實性構面,具有顯著性差異。結論:乳房X光巡迴車受限設備及空間都不如社區醫院,且因無預約制度,受檢者接獲衛生局通知後,又常相約一同前來檢查,同一時間湧進大量受檢者,造成等候時間過長,且無舒適的候檢室,所以滿意度得分較低。

According to the statistics of the National Health Service, there were 11817 new cases of breast cancer in 2014, ranking first in the incidence of cancer among women in Taiwan, and breast cancer also ranked fourth in the cause of cancer death in women. It shows that the breast cancer has a great impact on the health of women in my country. Because the age of breast cancer patients in Taiwan is about 45-64 years old, the Department of Health has reduced the breast cancer X-ray screening of women to 45 years old in 2010 to enable early detection of breast cancer in women. However, according to statistics, the proportion of women who do breast cancer screening in Taiwan is not as good as that of foreign countries. In order to increase the proportion of X-ray screening for breast cancer in women, six county and city health bureaus have provided digital mammography services in 2011 to promote breast cancer screening in the rural areas. Therefore, the mammogram tour vehicle becomes an important tool for breast cancer screening in Taiwan countryside. This study wanted to realize the difference in patient's satisfaction between mobile mammogram units and fixed mammography units. Methods: Using the service quality model proposed by Parasuram, Zeithaml and Berry scholars to develop the questionnaire which was designed with five dimensions of reliability, responsiveness, authenticity, care and tangibility. The survey objects were 167 women on breast X-ray screening in mobile mammogram units and 342 women in fixed mammography units were interviewed. Results: There was significant difference in authenticity and tangible facets. Conclusion: The limited equipment and space of the mobile mammography units are not as good as fixed mammography units. Because there is no reservation system, the examinee receives the notice from the health bureau and often comes together to check, and a large number of subjects are poured into the same time. The waiting time is too long and there is no comfortable waiting room, so the satisfaction score is low.
