照顧一位牙齦癌術後病人接受放射線及化學治療之護理經驗
Nursing Experience of Caring for a Post-Operative Gingival Cancer Patient Receiving Concurrent Chemoradiotherapy
摘要
本文描述照護一位女性罹患牙齦癌術後接受放射線及化學治療導致口腔黏膜炎、頭頸部皮膚潰爛、身體外觀改變等身心不適之症狀，及其心路歷程之經驗。護理期間自2013年10月18日至12月8日，經由直接照護、會談及觀察等方式收集資料，並運用Gordon 11項功能性健康型態進行評估，確立主要的健康問題：口腔黏膜改變、皮膚完整性受損及身體心像紊亂。運用護理專業及跨科部合作，提供個案及其家人在放射線皮膚炎及口腔黏膜炎的護理措施及知識，促進傷口的修復及提升自我照護能力；經由傾聽與鼓勵個案參與病友關懷團體、宗教信仰寄託，協助個案調適身心的痛苦及壓力，引導朝正向思考，使其順利完成治療。本文建議未來個案管理師在個案住院期間可及早介入且建立互相聯繫的管道，說明個案治療進度情形，也提供個案後續居家的照護，以維持良好的照護品質。透過此次的照護經驗，希望可提供臨床護理牙齦癌術後個案接受放射線及化學治療之護理參考。

The case study describes the nursing experience of caring for a patient with gingival cancer who received surgical intervention followed by concurrent chemoradiotherapy. The treatment often results in oral mucositis, skin ulceration on the scalp and neck regions, and an appearance change. Nursing should stress both relieving physical discomfort and treating psychological problems. From October 18th to December 18th, 2013, we collected data through direct nursing care, daily interviews and physical assessments, and evaluated patient's physical status with Gordon's 11 functional health patterns. The defined main problems included oral mucous membrane impairment, skin integrity impairment, and body image disturbance. We applied our nursing knowledge to care for oral mucositis and the skin ulceration. Furthermore, we educated the patient about how to enhance her self-care ability. In addition, we combined different types of expertise to offer comprehensive care to relieve the suffering of the patient and her family. Through listening to the patient's needs and encouraging positive thinking, the patient completed the treatment course smoothly. We highly suggest that the case manager should contact case case early when he or she is admitted, to follow up and provide continuous care information. We hope that sharing this nursing experience will be beneficial in caring for this kind of patient.
