Mobile Multimedia to Improve Self-Care Accomplishment Rate among Patients Receiving Chemotherapy at Home with Portable Delivery Pumps

摘要

背景：預防接受居家化學治療的病人發生異常狀況是護理師的重要任務，經現況分析攜帶式輸液幫浦居家化療病人之自我照顧完成率低僅67.2%，主要原因有：指導單張內容不足、指導方式不一致、口頭指導易疏忽部分內容、分段式護理指導、指導內容繁雜、講解速度快且術語多。目的：提升攜帶型輸液幫浦居家化療病人自我照顧完成率達90%以上。解決方案：實施策略經由建置行動多媒體影片、舉辦教育訓練、增修指導單張及居家電訪評值方式。結果：攜帶式輸液幫浦居家化療病人自我照顧完成率達99.1%，病人及家屬因指導方式及指導工具感到不滿意由10.6%降至0%，異常狀況病人及家屬執行後，無法排除事件由53件（2.28%）降為0件。結論：運用行動多媒體教學確實可提升護理師指導成效，增進居家化療病人自我照顧之效能。

Preventing abnormalities experienced by patients receiving chemotherapy at home is important for nurses. Current analysis shows that the self-care accomplishment rate among patients receiving chemotherapy at home with portable delivery pumps is low 67.2% mainly because of: 1. Insufficient information provided in the guidance leaflet; 2. Inconsistent guidance approach; 3. Ignorance of certain information provided during oral guidance; 4. Phased nursing guidance; 5. Complex guidance contents; and 6. Quick explanations with many terminologies. The goal was to improve the self-care accomplishment rate among patients receiving chemotherapy at home with portable delivery pumps. Implementation strategies included creating mobile multimedia videos, holding educational training, modifying the guidance leaflet, and evaluating home visits or telephone interviews. Results showed that the self-care accomplishment rate among patients receiving chemotherapy at home with portable delivery pumps reached 99.1%, and the dissatisfaction that patients and their family members had with the guidance approach and tool dropped from 10.6% to 0%. The number of adverse events decreased from 53(2.28%) to 0. By means of teaching through mobile multimedia, it can indeed make the guidance provided by nurses more effective and boost the efficacy of self-care among patients receiving chemotherapy at home.
