運用噪聲檢測行動應用程式改善腦中風中心加護病房之環境音量

Use of Mobile Noise Measurement Applications to Improve Ambient Noise in a Stroke Intensive Care Unit

背景: 環境音量過高影響病人預後及醫護人員工作表現。病家對單位環境安寧滿意度(平均91.9分),連4年低於全院平均值(92.7分),故成立專案小組。運用噪聲檢測行動應用程式測量發現高分貝、高頻率音量之主因為譫妄病人的吼叫聲、工作人員高音量談話;環境及設備缺乏介質;無機械設備使用規範、醫療儀器警示聲處理速度過長。目的:降低音量及頻率、提升滿意度與安靜感受度。解決方案: 藉教育訓練、「靜音速度王活動」、使用吸音材質及制定機械設備使用規範,以及噪聲檢測行動應用程式等措施。結果:音量平均由87.5降至69.0-70.0分貝,頻率由389降至289次(減少20%以上);病人及家屬滿意度大於93分,達專案目標。

Excessive ambient noise affects the recovery prognosis of patients and job performance of medical staff. A satisfaction survey revealed the patient's perceptions of the levels of noise in our unit (91.9), which were higher than the average of all units of our hospital (92.7) and remained higher levels for 4 years. Therefore, it was urged to start a quality improvement project. A mobile application for measuring noise levels was used. Our analysis showed that the main sources of high-decibel and high-frequency noise included loud voice from patients and medical staff, lack of soundproofing materials, lack of regulations for appropriate use of medical equipment, and prolonged alarm from monitoring devices. After the investigation, a 10-month improvement project was launched from January 2 to November 30, 2018 to decrease noise and increase patient satisfaction. The project consisted of education training and incentive programs to decrease ambient noise, adopt soundproofing materials, and develop mechanical equipment regulations. After the improvement project, the survey showed that the volume of ambient noise decreased from 87.5 dB to 69.0 dB, and the frequency decreased from 389 to 289 times (26% decrease). The volumes of five out of six main noise sources decreased to < 70 dB. The satisfaction scores of patients and their families improved to 93. The goal of this project was achieved.
