摘要

背景：急重症護理人員頭痛盛行率高達43.2％，理解其頭痛經驗可增進護理人員的健康。目的：探討護理人員頭痛影響其工作及生活之主觀經驗。方法： 以現象學研究法運用半結構式訪談指引，訪談北部某所醫學中心急重症單位患有頭痛的護理人員，依失能及疼痛程度分為九區塊，以立意取樣（purposive sampling）於每一個區塊，至少各選取三位護理人員訪談，資料以Colaizzi（1978）現象學方法進行分析。結果：訪談結果歸納三大主題為：一、與不確定性共存；二、徘徊不去的干擾及三、盡一切努力去忍耐或預防「這個不是病」的病。結論：不確定性為頭痛發作的本質，雖然護理人員試圖控制頭痛，但效果有限。建議經常頭痛的護理人員主動就醫，亦建議院方開設因應頭痛的課程，並提供護理人員頭痛發作時的彈性休息時間，以增進護理品質與提升病人安全。

Background: The prevalence of headache in critical care nurses is 43.2%. Understanding the experiences of nurses with headache could improve their wellbeing, care quality, and patient safety. Purpose: This study aims to explore the impacts of headaches on life and work in critical care nurses. Methods: With a phenomenological approach, in-depth semi-structured interviews were conducted among nurses with headaches. Purposive sampling was used to recruit in nine groups and include at least three nurses in each group according to different levels of pain intensity and disability. Participants were screened from critical care units at a medical center in Taiwan. Interviews were analyzed using the Colaizzi phenomenological approach. Results: ＂Controlling the uncertainty＂ was the essential phenomenon, and three categories emerged: 1. living with the uncertainty; 2. staying with the lingering interference; and 3. trying to prevent the attack ＂which is not seen a disease＂. Headache impacts multiple aspects of nurses' work and life. Conclusions: Uncertainty is the essence of a headache attack. Although nurses try to control their headaches, their coping behaviors are not always effective. Nurses experiencing frequent headaches should seek medical advice. To improve care quality and patient safety, hospital managers should hold the courses regarding managing headaches and allow flexible break times when nurses are suffering from a headache attack.
