運用翻轉教學提升護理師執行兒童中心靜脈導管護理之正確率

Using Flipped Classroom Teaching to Improve Accurate Execution Rate of Pediatric Central Venous Catheter Care by Nurses

摘要


背景:2018年初本單位發生中心靜脈導管血流重覆感染,且增加病童住院天數,故成立專案小組並自擬評核量表,評估相關知識正確率為92.3%,技能正確率為73.5%。執行正確率低的原因有:病童的哭吵或手足揮舞無法掌控、留置中心靜脈導管病童較少、缺乏兒童版中心靜脈導管護理技能評核標準、缺乏兒童版影音教材、無兒童中心靜脈導管照護課程、缺乏練習道具及無定期技術稽核制度。目的:提升護理師執行兒童中心靜脈導管護理之正確率。解決方案:舉辦在職教育課程、製作兒童版練習道具、拍攝「兒童中心靜脈導管傷口護理」教學影片,以翻轉教學模式進行學習及定期稽核,並上傳影片至院內學習平台。結果:專案實施後,知識正確率為100%,技能正確率為96.3%,達專案成效。結論:專案中所使用的翻轉教學模式,對於技能之教學策略效果良好,未來也建議用於其他臨床技術上之學習。

Background: In early 2018, repeated incidents of central venous catheter-related bloodstream infections increased the length of stay of patients. A project team was set up and designed a checklist tool to assess the accurate rate of knowledge (averaged 92.3%) and clinical skills (averaged 73.5%). The reasons for low accurate execution rates were uncontrolled movements and agitation of the children, a small number of patients with central venous catheters, lack of nursing skill standards in pediatric central venous catheter care, lack of video materials designed for pediatric patient groups, lack of courses on pediatric central venous catheter care, lack of practice tools, and lack of periodic assessment. Purpose: This project aims to improve nurses' accurate execution rate of pediatric central venous catheter care. Methods: The project team held in-service training courses, created pediatric practice tools, filmed a teaching video of pediatric central venous catheter nursing care, used flipped classroom teaching for periodic learning and assessment. The video was uploaded onto the hospital's online learning platform. Results: In the self-assessment after these implementations, the average score on the knowledge assessment was 100%, and the average score in clinical skills was 96.3%, demonstrating the efficacy of this project. Conclusions: The flipped-classroom teaching model used in the project is an adequate strategy for skill learning and is recommended for other clinical skills teaching in the future.
