照護一位女性口腔癌病人接受手術之護理經驗
Nursing Experience of Caring for a Female Patient with Oral Cancer Who Underwent Surgery
摘要
本文內容為一位女性口腔癌病人接受腫瘤切除及皮瓣修補手術前後之護理經驗。護理時間自2018年11月22日至11月26日及2018年12月3日至2019年1月14日，透過筆談、會談、觀察、身體評估及查閱病歷等方法收集資料，從生理、心理、社會及靈性四個層面進行護理評估，發現個案有急性疼痛、吞嚥障礙、身體心像改變等健康問題。護理期間透過藥物及非藥物方式來降低疼痛情形、給予吞嚥訓練及提供適合的進食工具，幫助個案由鼻胃管灌食成功轉換為由口進食，並主動陪伴、傾聽、給予正面肯定，成功引導個案接受顏面外觀改變的事實。建議對於顏面外觀改變的病人，尤其是遇到注重自身儀容之病人，護理師在照護上除了協助緩解生理的不適之外，也應特別注重病人的心理層面，幫助他們重建對外觀改變的正向態度，順利出院回歸社會家庭。希望透過此個案之照護經驗，提供日後護理同仁照護類似病人之參考。

This article describes the experience of providing nursing care for a female patient with oral cancer before and after surgery. Information regarding physical, psychological, social, and spiritual aspects of nursing assessment were collected through direct care provision, interviews, written exchanges, observations, physical examinations, and medical chart reviews from November 22-26, 2018, and December 3, 2018, to January 14, 2019. The client's health problems were acute pain, impaired swallowing, and body image changes. During the process, we provided drug-based and non-drug-based methods to relieve pain and training for swallowing and using suitable feeding tools to convert from nasogastric tube feeding to oral intake. By accompanying, listening to, providing positive affirmation, we guided the patient to accepting the facial changes. We prompted the patient to learn responding to the effects of the disease. We believe this experience will help other nursing staff cope with challenges such as body image disturbance after oral surgery and improve physical, mental nursing care.
