肝硬化末期病人等待肝臟移植之護理經驗

本文主要在探討一位肝硬化末期合併腸胃道出血個案等待肝臟移植的照護經驗。照護期間為2018年8月1日至8月21日，運用會談、身體評估、觀察及病歷查閱收集資料。以Gordon 11項功能性健康型態進行護理評估，確立個案健康問題有疼痛、無望感、主要照顧者角色緊張。在護理過程中與個案及主要照顧者建立良好護病關係，教導個案及主要照顧者使用精油穴道按摩及聆聽音樂，降低疼痛；鼓勵適當表達自己想法及情緒發洩，面對疾病能自我調適及肯定自我存在價值，召開醫療團隊會議，說明治療計畫與目的，分享肝臟移植成功案例，增加個案及家屬的信心及降低其焦慮與不安。建議未來增加肝病個案管理師，將住院中肝硬化病人分級並列入名冊定期追蹤，舉辦衛教講座，衛教保持規律生活作息及飲食原則，延緩肝硬化短期內快速進展，促使後續照護計畫更完善。

This article describes our experience of nursing a patient with end-stage cirrhosis with gastrointestinal bleeding who was waiting for a liver transplant to survive. The period of nursing care was from August 1, 2018, to August 21, 2018. The relevant data were collected through interviews, clinical assessments, and chart reviews. The patient's health problems were identified using Gordon's 11 Health Functional Patterns, including pain, hopelessness, and caregiver role strain. The authors established a trustful relationship with the patient and her caregiver and educated them about acupuncture point massage with essential oils and listening to music for pain relief. The authors also encouraged them to express their thoughts and emotions openly and to participate in the meeting with the medical team, understanding the treatment plans and goals. The authors also shared successful liver transplantation stories to increase the patient's confidence and reduce anxiety. The authors suggest that more case managers may be needed to promote the health care quality through regular patient education lectures, close follow-up, and adequate monitoring plan according to disease severity.
