Fulminant Type 1 Diabetes Mellitus: Report of Two Cases

Background. Diagnosis of fulminant type 1 diabetes, a novel subtype of type 1 diabetes mellitus, is currently of considerable challenge to physicians. Without prompt treatment, fulminant type 1 diabetes may become life-threatening. Methods. We reports two cases with the diagnosis of fulminant type 1 diabetes in our hospital. Results. Each of the two patients presented with rapidly progressive hyperglycemia, severe ketoacidosis and hyperkalemia. Other laboratory investigations revealed near-normal HbA_(1c) values and absence of the islet autoantibodies. Conclusion. Fulminant type 1 diabetes is not uncommon in the Asian population, and its clinical course is rapidly progressive and severe. Therefore, clinicians should be alert for its presentation such that the correct diagnosis is made and appropriate treatment administered as rapidly as possible.
