Efficacy of Virtual-Reality Walking on Patients with Schizophrenia: A Pilot Randomized Controlled Trial

Background: Schizophrenia patients have poor physical fitness, which might lead to decreasing psychosocial wellbeing and increasing risk on cardiovascular diseases. The present study would focus on the efficacy of virtual-reality combined with treadmill walking for quality improvement on the population. Objective: The aims of this study were to find out the efficacy on physical fitness and psychosocial function under virtual-reality based treadmill walking on schizophrenia patients. Methods: This study was a single-blind randomized controlled trial. The intervention group conducted virtual-reality based treadmill walking activities for 4 weeks. All participants underwent pretest, posttest and follow-up evaluations of physical fitness and psychosocial function. Results: The intervention group had significantly greater improvement than control group on walking distance (p=.006), lower-extremity endurance (p=.026) and self-efficacy (p=.041). The walking activities led to significantly higher scores on walking distance (p=.025) and lower-extremity endurance (p=.017) than control group from posttest to follow-up, providing the evidence on the efficacy of physical fitness under continued intervention can be at least maintained for 1 month. Conclusions: The preliminary results supported that virtual-reality based walking activities can improve the physical fitness, lower-extremity endurance and self-efficacy among schizophrenia population.
