以環保活動為基礎結合復健運動模式導入社區關懷據點對高齡者身體活動功能表現之影響:先驅性研究

Physical Performance Improvement in Community-Based Recycling Program Combined with Physical Rehabilitation in Older Adults: A Preliminary Study

摘要


背景與目的:「社區照顧關懷據點」(簡稱社區據點)為政府因應國人高齡化衝擊所提出以社區照顧為基礎,落實在地老化的長照策略方式之一。先前研究未探討以環保活動結合復健運動模式導入社區據點對高齡者活動功能之影響。方法:本研究採單一樣本前後設計,收集花蓮某社區據點長者共23位。長者參與社區據點每周兩次環保活動搭配一次復健運動指導與諮詢,每次兩小時,連續12周。環保活動包含紙類書籍,塑膠製品,瓶罐類,廢棄五金電器,及其他如舊衣物等之回收分類與處理。一位未參與運動指導之物理治療師於研究前與後一週內執行握力(grip strength),坐站五次(five times sit-to-stand),柔軟度(flexibility),單腳站立(single-leg-stance),計時起走測試(Timed-up-and-go test)與走路速度(gait speed)等活動能力(physical mobility)評估。結果:20位老人完成本研究,平均年齡76.1±6.2歲(男性5位,女性15位),參與率88±8.7%。參與者後測除單腳站表現前後無顯著變化外,在其他活動能力項目均有顯著改善。結論:本研究初探得知每周三次、每次2小時,持續3個月的環保活動加復健運動指導與諮詢於社區據點之可行性,並對社區長者之身體活動功能有所助益。

Objectives: In response to the ever-increasing presence of aging societies worldwide, the Taiwanese government has developed more than 3000 community-based care centers (CBCCs) providing various programs to help achieve the goal of "aging in place" for older adults. Among the provided community programs, the efficacy of the recycling program combined with physical rehabilitation has not yet been verified. Methods: A Preliminary study with the single-group pretest-posttest design was conducted with 23 elderly subjects recruited from a CBCC in Ji'an Township, Hualien. All subjects participated in a 2-hour recycling program twice a week for 12 weeks, sorting and handling waste paper products, plastics, metals, electric/electronic products, and clothes. A 2-hour weekly rehabilitation exercise program was integrated into the recycling program. A physical therapist evaluated grip strength, the five-times-sit-to- stand test (FTSST), flexibility, one-leg-stance (OLS), Timed Up and Go (TUG) and gait speed before and after the study. 20 participants (aged 76.9±6.3 years, M=5, F=15) completed the study with adherence 88±8.7%. Results: Significant improvement was detected in grip strength, FTSST, flexibility, TUG, and gait speed after the program, while there was no significant change in OLS. Conclusions: Our preliminary study suggests that the combination of recycling activities and rehabilitation program at community-based care centers has potential as a sustainable and feasible model to improve physical performance of older adults in Taiwan. However, further studies with randomized control trials are needed to confirm the efficacy of recycling program combined with different interventions.
