因廣泛性化膿性乾癬住院後引發之功能衰退

台灣老年人口比例迅速增加，老年人口住院佔率也逐年增加。老年人因急性疾病住院後，常發生功能衰退甚至失能的情形，不但增加家庭與社會照護上的負擔，也影響老年人的生活品質。本文個案為一位因廣泛性化膿性乾癬住院的91歲女性，因住院期間功能衰退而在住院後被送至養護機構。個案的功能在養護機構沒有得到改善，因而產生憂鬱情緒。老年科跨專業團隊透過周全性評估發現個案問題，提供合適輔具進行復健活動，了解其憂鬱原因給予情緒支持，協助個案逐漸恢復功能，獲得較好的生活品質。

In Taiwan, the number of individuals aged ≥65 years has increased rapidly, triggering a corresponding rise in the number of elderly people hospitalized. For elderly patients, an acute illness that requires hospitalization often causes rapid functional decline or even disability, which subsequently exacerbates family and societal burden and results in poor quality of life for affected individuals. In the article, we report the case of a 91-year-old woman hospitalized for generalized pustular psoriasis. As she underwent functional decline during hospitalization, she was moved to a nursing home after discharge. She started to experience depression at the nursing home as there was no functional improvement. Through comprehensive geriatric assessment, our transdisciplinary team studied and managed her problems through rehabilitation using an appropriate assistive device and provision of emotional support in response to her depression. Thereafter, the patient's function and quality of life gradually improved.
