斷鏈防疫～建置COVID-19社區篩檢站經驗分享

目的：COVID-19的快速檢測對於遏制大流行和安全地重新開放社會有所助益。個案醫院為配合防疫政策，必須成立社區篩檢站，期間發現缺乏採檢設施、動線規劃、推動組織、作業規範、人員把握度低等，而且人員個人防護設備穿脫正確率為78.5%，期望設置篩檢站儘早發現確診者，阻斷傳播鏈，防堵疫情繼續蔓延，以達全民均健。方法：2021年6月03日至6月25日，分計畫期、執行期、評值期三個階段，運用世界衛生組織渥太華健康促進憲章中的五大行動綱領的原則，經實地勘察、查核等方法收集資料及擬定改善策略，包含成立推動小組、建置作業規範、line群組、支援人力機制、訓練機制、採檢設施及動線規劃等。結果：社區篩檢站每日服務達成率平均90.3%、快篩陽性PCR陰性個案通知率100%、個人防護設備穿脫正確率由78.5%提升至100%。結論：齊心防疫策略除勤洗手、戴口罩及社交距離外，產官合作成立社區篩檢站，有利防疫政策的可近性，阻斷潛藏在社區的感染源，建議可以分區建置，在防疫熱區多增設社區篩檢站，非熱區則將篩檢站就近建置於轄區醫院或衛生所，以達管理效益。

Objectives: The COVID-19 rapid antigen test is a helpful tool to stop the pandemic and safely reopen communities. For the case hospital to adhere to the anti-epidemic policy, we quickly set up a community screening station. We found a lack of essential equipment and inspection facilities, route planning, promotional organization, operation specifications, and low personnel awareness, etc. Also, the percentage of staff correctly employing personal protective equipment was only 78.5%. We hoped that screening stations would be set up to detect confirmed cases as soon as possible, block the chain of transmission, prevent the spread of the epidemic, and deliver healthcare for all. Methods: It was divided into three stages: planning, implementation, and evaluation, from June 03 to June 25, 2021. We apply the principles of the Five Action Programs in the Ottawa Health Promotion Charter of the World Health Organization, collected data and formulated improvement strategies through on-site surveys and inspections. For example, we established a promotion group, operating standards, Line social groups, support workforce mechanisms, training mechanisms, inspection equipment, route planning, etc. Results: Achieved on average 90.3% of daily service achievements for community screening stations, 100% of PCR negative case notifications, and 100% adherence to correctly-equipped protective practices, elevated from 78.5% to reach our project goals. Conclusions: To build an healthy society, we would like to focus on epidemic prevention strategies such as frequently washing hands, wearing masks, and maintaining social distancing. The establishment of community screening stations through cooperation between production and government officials is conducive to the accessibility of the epidemic prevention policy, as it blocks the sources of infection hidden in the community. Therefore, it is recommended that they be established in districts, and more community screening stations should be established in epidemic prevention hot spots. Non-hot spot areas will also be screened. Checkpoints should be built in the nearest hospitals or health centers in the jurisdiction to achieve management benefits.
