因應新冠肺炎第一線呼吸治療師防疫管理及作為－以南部某區域教學醫院為例

摘要

臺灣歷經2003年新興傳染病嚴重呼吸道症候群（Severe Acute Respiratory Syndrome, SARS），爆發SARS疫情，曾造成嚴重的社會恐慌與傷害，在缺乏配套措施與足夠的設備同時，造成整個防疫體系瞬間崩解。文獻回顧指出新冠肺炎（Coronavirus disease 2019,COVID-19）病人中20.3%需要入住加護病房，而入住加護病房的病人中，約75%需要使用呼吸器。第一線面對高風險挑戰即為呼吸治療師，呼吸治療科透過協調整合防疫管理，於疫情衝擊前提早啟動規劃、早期部署及透過全員能配合相關防疫措施。當醫院進行服務量降載時，呼吸治療科妥善運用人力，制訂分棟、分組、分層、分區作業標準，在臨床照護面、物資設備面、管理流程面等，明訂疑似或確診病人處置流程及感染管制措施杜絕院內感染。在防疫物資、臨床處置及人員疫苗接種之防疫管理，維持呼吸治療服務量能正常運作，全員抗疫有成，期能提供相關經驗分享。透過結構化整合及標準作業流程，讓全科呼吸治療師都能了解感染管制及防疫目標的重要性，以達到防疫終極目標及管理作為。

In 2003, Taiwan experienced the Severe Acute Respiratory Syndrome (SARS), an emerging infectious disease. The outbreak of SARS caused damaging social panic. The lack of supporting measures and adequate equipment caused the entire epidemic prevention system to collapse quickly. Literature review stated that 20.3% of patients with severe special infectious pneumonia (Coronavirus disease 2019, COVID-19) needed to be admitted to the intensive care unit. Of these patients admitted to the intensive care unit, 75% needed to use respirators. The first line of high-risk challenges is the respiratory therapist. The respiratory therapist department coordinates and integrates epidemic prevention management, initiating early planning and early deployment before any epidemic occurs, and staff can cooperate with relevant epidemic prevention measures. When the hospital performs service load reduction, the respiratory therapy department makes proper used of manpower to formulate operating standards for building division, grouping, stratification, and zoning. This clearly stipulates the treatment of suspected or confirmed patients in clinical care, materials and equipment, management procedures, etc. These procedures and infection control measures prevent nosocomial infections. The epidemic prevention management of epidemic prevention materials, clinical treatment and personnel vaccination, the maintenance of respiratory treatment services can operate normally, and all employees are successful in fighting the epidemic. We hope to share relevant experience. Through structured integration and standard operating procedures, general respiratory therapists can understand the importance of infection control and epidemic prevention goals, as to achieve the ultimate goal of epidemic prevention and management.
