COVID-19疫情影響醫療策略聯盟架構下醫師與醫院合作意願之探討

目的:國內有85%的醫院有策略聯盟合作機制,合作利益與疫情風險將影響醫院與醫師對於醫療合作的想法,本研究擬探討策略聯盟醫院與醫師面對COVID-19疫情如何影響雙方之合作意願。方法:本研究調查期間為109年3月23日至4月17日,以問卷形式調查有策略合作的47間醫院與91位醫師對於防疫措施滿意度與不同疫情下合作模式因應措施,問卷結果使用SPSS軟體進行描述性統計、單因子變異數分析、皮爾森相關係數進行分析。結果:合作醫院自評防疫信心(4.63±0.49)高於合作醫師(3.91±0.72),合作醫師對醫院的防疫信心程度越高越願意繼續合作,相關係數為0.746;6成的醫師認為當地社區發生感染應停止合作,7成的醫院認為發生該院發生院內感染才需停止。結論:醫院在防疫時應將合作醫師視同正式員工管理,避免成為防疫的漏洞;亦建議政府可善用既有醫療策略聯盟合作網絡的力量發揮區域聯防的加乘功效。

Objectives: 85% of hospitals in Taiwan have joined the strategic alliance. The benefits of cooperation and risk of the pandemic will affect the hospital's and physicians' willingness about medical cooperation. This study intends to explore how the strategic alliance hospitals and physicians will affect their willingness to cooperate in the face of the COVID-19 pandemic. Methods: The survey period of this research was from March 23 to April 17, 2020. The questionnaire surveyed 47 hospitals and 91 doctors who strategically cooperated on the satisfaction of epidemic prevention measures and response measures of cooperation modes under different epidemics. The results of the questionnaire were analyzed using SPSS software for descriptive statistics, single-factor analysis of variance, and Pearson's correlation coefficient. Results: The self-assessed confidence in epidemic prevention of cooperative hospitals (4.63 ± 0.49) is higher than that of cooperative physicians (3.91 ± 0.72). The higher the degree of confidence of cooperative physicians in epidemic prevention in the hospital, the more are willing to continue cooperation, with a correlation coefficient of 0.746. 60% of physicians believe that cooperation should be stopped when infection occurs in the local community, and 70% of hospitals believe that nosocomial infection in the hospital needs to be stopped. Conclusions: Hospitals should treat cooperating physicians as regular employees during epidemic prevention to avoid becoming a loophole in epidemic prevention; it is also recommended that the government make good use of the strength of the existing medical strategic alliance cooperation network to give full play to the effect of regional joint defense.
