救命還是自救?醫療商業化下的醫師與醫美產業

Saving People or Saving Themselves? The Doctors and Medical Cosmetology Industry of Medical Commercialization

摘要


愛美是人的天性?還是社會型塑的需求?不論如何,這都開創了龐大的美麗商機。非必要性醫療的美容醫學介入,讓人擁有更多改變的可能。另一方面,也反映出醫療商品化之下,醫療體制的變遷。當醫療社群將「救美不救命」視為職涯出路,是否也代表醫療環境出了問題? 本研究透過文獻回顧與資料整理,歸納發現,美容醫學場域的行動者多元且複雜,包含廠商、醫學會、醫師、診所、消費者、政策等。醫療專業正被商業力量慢慢滲透,美容醫學正是醫療服務商業化的最佳案例。社會學理上醫療專業化、生物醫療化與商品化的脈絡,更提供後續報導相關的背景資料。在市場領導下,愛美的需求變成醫療問題,健保制度的限縮,讓自費市場更成為醫療機構及醫師都想分一杯羹的獵物。然而,連鎖集團醫美診所企業化的經營管理,以及美容醫學在專業上的空白,卻讓醫師在執行醫療業務時,承受更多未知的醫療風險,甚至影響消費者的健康。 報導以幾位醫美醫師故事為主軸,呈現醫師在不同世代、不同執業型態的選擇與價值觀,輔以醫美診所經營者對產業的思維,提供讀者另外一種思考層面:不論健保或自費醫療,都面臨商業化的轉型,非必要性醫療的介入,擴充了醫療範疇,看似為醫療社群打開另一扇窗,實際上,卻既得利益者卻並不一定是醫療社群中的多數醫師。 再者,完全交付市場機制的醫美產業,不如想像中光鮮亮麗,也不是每個醫師都適應非健保醫療的市場思維;站在醫美診所經營者角度,醫美醫師替代性高,一旦市場消費力下降,醫師勞動條件也可能面臨削價競爭。商業化醫美削弱醫師原有的專業價值與醫療權威,醫師反而成為商業操作中的棋子。

Is pursuing beauty a human nature? Or the psychological needs shaped by the society? Either way, this is creating a business opportunity. People regard medical cosmetology as a product. We can get pretty through surgery. In the same time, medical cosmetology brings unnecessary medical risk. National Health Insurance policy change medical environment, effect physicians’ working, psychology and life quality. Medical cosmetology industry seems to be a brilliant broad road. Under this frame, more and more physicians give up their subject, turn to be the medical cosmetology physicians. Is it really a better way of their career? Through the literature review and in-depth interview, I try to present the value and the position of medical cosmetology physicians in the medical commercialization. The report shows unnecessary medical intervention seemingly expand health care system, provide another window for medical community. In fact, it’s not good for majority of them. Commercialization give no professional autonomy to physicians, highly diminishes the physicians’ specialty, expertise and dignity. Furthermore, the market mechanism control the medical cosmetology industry, not every physician can adapt market thinking. In clinic operator perspective, medical cosmetology doctors are replaceable. Once recession occur, the doctors’working conditions may also face price competition.
