具連續血壓量測與即時回饋控制系統下肢加壓循環器之研發

摘要


因脊髓損傷而產生姿勢性低血壓的患者在實施傾斜床訓練時,因傾斜床角度上升使血液積存於下肢,導致腦部血流量減少,因而產生血壓下降、眼花、暈眩等症狀,稱為姿勢性低血壓。本研究設計一即時回饋控制系統來控制下肢加壓循環器,可即時偵測姿勢性低血壓的發生,自動啟動加壓裝置以幫助血液循環。 本研究使用脈波抵達時間作為參數推估連續血壓,可以取得患者連續血壓資訊。當傾斜床訓練時,且患者血壓下降幅度超過20mmHg時,控制系統將自動啟動下肢加壓循環器,幫助血液回流,經過臨床實驗結果,發現當下肢加壓循環器啟動後,血壓會有回升的趨勢,冀對未來姿勢性低血壓患者作傾斜床訓練時有所幫助。

Patients who suffer from spinal cord injury (SCI) usually have the symptoms of postural hypotension. As the tilt table rises, blood still remains in lower extremities which will cause dizziness and syncope. This research applies a real-time feedback control on Circulator Pumping System of Lower Extremities (CPSLE) which can immediately detect the symptoms of postural hypotension and automatically start CPSLE to help blood circulation. In this research, pulse arrival time is used as a parameter to estimate continuous blood pressure. If the patient’s blood pressure decreases more than 20 mmHg during tilt table training period, the CPLSE will automatically start to help blood circulation and raise the blood pressure.
